摘  要:目的探讨实施阶段性责任制健康教育对提高产科质量的促进作用。方法针对孕产妇的心理需求,制定产妇住院期间不同阶段的健康教育内容以及产科健康教育实施表,再造宣教流程,选择2009年2—6月的住院产妇1500例为观察组。选择2008年6月—2009年1月住院的产妇1500名为对照组,按照常规进行产科健康教育,对其住院资料进行分析。观察组在住院的全过程责任到人,落实和接受情况由宣教者及受教育者双方签名,并采用自行设计的产妇住院全程意见征询表进行调查,对阶段性责任健康教育的各项内容掌握程度进行评估,比较2组产妇相关知识及技能掌握情况,护患纠纷发生率、护理投诉及护理满意度。结果观察组产妇健康教育知识中的合理饮食、自我监护、分娩时配合母乳喂养技巧、伤口护理、婴儿护理、异常识别掌握情况明显优于对照组,差异均有统计学意义(χ2=58.01、94.28、94.73、37.50、58.36、41.04、91.34,均P<0.01)。观察组无护理投诉、纠纷发生,满意度达(98.66%),与对照组比具差异有统计学意义(P<0.01)。结论围生期应用阶段性责任制健康教育可显著提高教育质量和效果,提升母婴生活质量,在临床实践中切实可行,值得推广。[Objective]To study the promotion of the implementation of the stages responsibility health education on obstetric quality improvement.[Methods]1500 puerperas hospitalized from February to June 2009 were selected as the observation group which was given the stages responsibility health education. According to the psychological needs of pregnant women,contents of health education in different stages in the hospital and implementation table of obstetric health education were established,and the education process was reforged. 1500 puerperas hospitalized from June 2008 to January 2009 were selected as the control group which was given routine obstetric health education,and their in-hospital data were analyzed. In the observation group,designated nurses were responsible for the whole process,implementation and acceptance situation were confirmed by the signature of educator and educate. The self-designed questionnaire for opinions of puerperas in the whole process was adopted,and the awareness level of contents of the stages responsibility health education was assessed. The awareness of related knowledge and skill,nurse-patient dispute rate,nursing complaint and nursing satisfaction of two groups were compared.[Results]The awareness of rational diet,self-monitoring,breastfeeding technique,wound care,baby care and anomaly identification among puerperas in the observation group were better than that in the control group significantly ( χ2 = 58. 01,94. 28,94. 73,37. 50,58. 36,41. 04,91. 34,all P 0. 01) . There was no nursing complaint and dispute in the observation group,the nursing satisfaction reached 98. 66% ,and the difference was significant between the observation group and the control group. [Conclusion]Implementation of the stages responsibility health education during the perinatal stage can improve the quality and effect of health education significantly,enhance the life quality of maternal and infants. It is feasible in clinic,which is worth promotion.
