白内障超声乳化术后角膜水肿的护理

摘　　要：目的:减少老年性白内障超声乳化术后的并发症。方法:对897例957眼老年性白内障手术病人加强术前心理护理和宣教,术中配合,术后角膜观察、作好专科护理及出院指导。结果:297眼老年性白内障手术病出现角膜水肿,其中,Ⅰ级角膜水肿238眼,2天内全部恢复透明,Ⅱ级角膜水肿45眼,4天内消退恢复透明,Ⅲ级角膜水肿10眼,6～10天内消退恢复透明,Ⅳ级角膜水肿4眼,10～25天内消退恢复透明,14眼重度角膜水肿者晶体核硬度均为Ⅴ级核。结论:晶体核硬度与术后角膜水肿的轻重密切相关,做好术前护理、术中配合、术后作好裂隙灯下角膜观察及护理,有利于手术后视力的提高,防止大泡性角膜病及角膜变性的发生。Objective：To reduce the complications after ultraemulsification in senile cataract patients.Methods：A total of 957 eyes of 897 senile cataract patients were selected.All patients were strengthened on the preoperative psychological care,propaganda and education.The intraoperative coordination was implemented and cornea was observed after operation.The patients were given good special care and discharging guide.Results：297 eyes developed corneal edema in different degrees.Among them,238 eyes had Ⅰlevel corneal edema and recovered to transparency within 2 d.77 eyes had Ⅱ level corneal edema and recovered to transparency within 4 d.10 eyes had Ⅲ level corneal edema and recovered to transparency within 6-10 d.4 eyes had Ⅳ level corneal edema and recovered to transparency within 10-25 d.14 eyes with severe corneal edema had V grade hardness of lens nucleus.Conclusion：The hardness of lens nucleus is closely correlated with the postoperative corneal edema.Therefore,good preoperative nursing,intraoperative coordination and corneal observation and nursing by slit lamp are conducive to the improvement of visual acuity after surgery and prevent bullous keratopathy from happening.
