碱性成纤维细胞生长因子和硫糖铝促进扩张头皮生长的临床应用

摘　　要：目的观察碱性成纤维细胞生长因子(bFGF)和硫糖铝促进头皮扩张的临床效果。方法 26例患者共埋置扩张器62个,随机分为治疗组和对照组,采用自身对照。治疗组在持续恒压扩张的同时于头皮埋置的扩张器表面与组织间注入bFGF和硫糖铝,对照组仅持续恒压扩张。术后第2天开始扩张,观察扩张效果、皮瓣近远期成活情况、秃发区消除情况、毛发生长密度和外观,测量即时回缩率。结果治疗组平均扩张时间、皮瓣即时回缩率较对照组明显减少(P<0.05),无明显并发症发生。修复创面后皮瓣血运良好,秃发区消除,毛发生长佳。结论 bFGF和硫糖铝局部联合应用,是促进头皮扩张的一种较理想的方法。Objective To observe the clinical effect of bFGF and sucralfate during scalp tissue expansion. Methods62 expanders in 26 patients were selected self-controlled in the study,and divided into treatment group and control group randomly. Scalp tissue expansion combined with local administration of bFGF and sucralfate was used in 26 patients and continuous tissue expansion only in control group. Continuous expansion began at 2 days af-ter expander implantation. Postoperative complications, repairing effect were observed. The immediate stretch-back rate of the expanded skin was detected. The elimination of alopecia area, hair density and appearance were detected during follow-ups.ResultsIt reduced of the average inflation time and the average immediate stretch-back rate of the expanded skin in treatment group comparing with them in control group （P〈0.05）. The clinical results were sat-isfactory without any complications. Psilotic scar eliminated in all cases. It was found by a postoperative follow-up of 1～3 years that hair growth was good.ConclusionLocal application of bFGF and sucralfate during scalp tissue ex-pansion is better method. It could accelerate skin expansion and reduce the retraction rate of expanded skin.
