摘  要:目的观察骨保护素(OPG)全段甲状旁腺激素(iPTH)在慢性肾脏病(CKD)中的变化以及与骨密度(BMD)的相关性。方法采用ELASA法测定100例慢性肾脏病患者和健康体检人群的血清OPG和iPTH值。采用双能X线骨密度仪测定上述人群的1~4腰椎前后位BMD和T值。结果 CKD5期(Ccr<15mL/min)T值和BMD值与体检人群比较差异有统计学意义(P<0.05);CKD2~4期T值和BMD值与体检人群比较差异无统计学意义(P>0.05)。CKD3~5期患者血清OPG显著高于体检人群(P<0.05),CKD4、5期患者血清iPTH明显高于体检人群(P<0.01)。相关分析表明OPG和iPTH与骨密度相关。结论 CKD患者OPG和iPTH改变早于BMD和T值改变。Objective To observe the variances of Osteoprotegerin (OPG) and intact Parathyroid hormone (iPTH) in chronic kidney disease (CKD) patients. And to elucidate the relationship between the serum OPG and PTH with BMD. MethodsThe serum concentrations of OPG and iPTH in normal subjects and 100 CKD patients were measured using ELISA The lumbar BMD (L1~4) were measured using dual X ray absorptiometry (DEXA) in 100 CKD patients and 25 age-matched controls. ResultsThe BMD and T-score in CKD stages 5 were significantly lower than those of the sex-age matched group(P 0.05). The serum concentrations of OPG in patients of CKD stage 3~5 were higher than those of the controls significantly (P 0.05); Compared to controls, iPTH level in patients of CKD stage 4~5 was significantly elevated. ConclusionsSerum OPG and iPTH changed early than BMD in CKD patients.
