摘　　要：目的探讨门静脉系统三维影像重组下腹腔镜选择性断流术治疗门静脉高压症的效果。方法分析2007年1月～2009年8月35例门静脉高压症患者门静脉三维影像重组下行腹腔镜选择性断流手术治疗的临床资料,分析其疗效。结果 35例患者随访8～37个月,无手术死亡病例,术后无近期出血和肝性脑病发生。食管静脉曲张明显改善。结论选择合适病例行门静脉系统三维影像重组下腹腔镜选择性断流术是安全有效的,门静脉三维影像重组对选择断流手术方式有重要价值。Objective To assess the value of 3D-CT or MRA imaging to display whole portal venous system in laparoscopic selecting disconnection of portalazygous venous collaterals. MethodsAnalyzed the results of 35 patients with liver cirrhosis, portal hypertension who were treated with endoscopic variceal ligtion plus 3D-CT or MRA imaging to display whole portal venous system in selecting disconnection of portalazyous veous collaterals.Results All the patients were followed up from 8-37 months. There was no operative mortality, no short term rebleeding and no encephalopathy after operation. Conclusion3D-CT or MRA imaging to display whole portal venous system in laparoscopic selecting disconnection was feasible,effective,safe method with advantages in treating portal hypertension. There is great value of three-dimensional imaging based on 3D-CT or MRA in portal venous system to select operation.
