摘　　要：目的:探讨不同证型特发性血小板减少性紫癜(ITP)患者糖皮质激素受体(GR)α和β亚型的表达水平及生血灵的调节作用。方法:应用免疫细胞化学方法分别检测脾肾阳虚型ITP和脾肾阴虚型ITP患者在生血灵治疗前后GRα和GRβ的免疫阳性的外周血单个核细胞(PBMC)数目的变化,并和正常对照组比较。结果脾肾阳虚型和脾肾阴虚型ITP患者GRα免疫阳性PBMC数与正常对照组比较明显下降(P<0.05),但两型之间比较差异无统计学意义;脾肾阴虚型ITP患者GRβ免疫阳性PBMC数明显高于脾肾阳虚型(P<0.05);经生血灵治疗后患者GRα数上升,GRβ数下降。结论:脾肾阴虚型ITP患者GRβ数较脾肾阳虚型明显升高,GRβ可作为脾肾阴虚型ITP辨证分型的参考指标;生血灵治疗ITP能调节GRα和GRβ的比例失衡。Objective：To study on the level of glucocorticoid receptor-？？ GR？？ ,glucocorticoid receptor-？¨ GR？？ expression in different syndrome types of traditional Chinese medicine（ TCM） in patients with idiopathic thrombocytopenic purpura（ ITP） and the regulation treated with Shengxueling. Methods：Patients with ITP were classified as the deficiency of Spleen and Kidney-Yang type and the deficiency of Spleen and Kidney-Yin type and normal peoples as control,and peripheral blood mononuclear cells （ PBMC） were isolated. The immunocytochemical assay was used to investigate the expression of GR？？and GR？ in PBMC befor and after the treatment with Shengxueling. Results：The PBMC number of GR？？of the deficiency of Spleen and Kidney-Yang type and the deficiency of Spleen and Kidney-Yin type decreased significantly than the control group（ P 0. 05） ,but there were no significant difference between them; the PBMC number of GR？ of the defficiency of Spleen and Kidney-Yin type is much higher than the deficiency of Spleen and Kidney-Yang type（ P 0. 05） ; the number of GR？？raised and GR？ decreased after the treatment with Shengxueling. Conclusion：The number of GR？ of the deficiency of Spleen and Kidney-Yin type is much more than the deficiency of Spleen and Kidney-Yang type,the GR？ is thought as the micro factor of the deficiency of Spleen and Kidney-Yin type that is valuable for the differentiation type of TCM. Shengxueling can regulate the disproportion of GR？？and GR？ expression in patients with ITP.
