开放复位治疗跟骨关节内骨折51例

摘　　要：目的探讨开放复位治疗跟骨关节内骨折的效果。方法在排除足骨筋膜室综合征后,于肿胀减轻后10～12d行择期手术,均在腰硬联合麻醉下完成手术,依次整复跟骰关节面、Bohler角、Gissane角、跟骨高度和宽度及距下关节面,矫正内外翻畸形,内固定物固定,术毕确定手术效果满意后放置引流管,缝合,术后消炎消肿处理,随访1年观察并记录手术效果,有效率=优+良。结果治疗后Creigton-Nebraska评分,SandersⅢ型的优良率为89.7%,未见Creigton-Nebraska评分<50分,疗效差者,SandersⅣ型的优良率为87.0%,疗效中和差者各1例,两型患者手术治疗后效果比较没有差别,1例SandersIV型患者术后出现腓肠皮神经损伤症状,1例SandersIV型患者存在创伤性关节炎表现。结论切开复位治疗跟骨关节内骨折可取得满意的结果。Objective To explore the open reduction and treatment of the effect of intra-articular calcaneal fractures. Methods The exclusion of foot compartment syndrome,after injury to reduce swelling after 10 to 12 days for elective surgery,they were in the lumbar epidural anesthesia completion of surgery,followed by restoration of articular surface with the dice,Bohler angle,Gissane angle,height and width of the calcaneus and subtalar joint surface,correct varus deformity,implant fixation,results of operations of surgery to determine the satisfaction of simple drainage, suture, swelling anti-inflammatory treatment after office followed up 1 year. Observe and record results of operations,efficiency--Excellent ＋Good. Results After treatment use the Creigton-Nebraska score,Sanders m type of good was 89.7%,with no Creigton-Nebraska score〈50,effective in poorer,Sanders 1V type of good rate was 87.0%,of the efficacy and poor in 1 patient,after surgical treatment of patients with type 2 comparison has no difference.One case of postoperative patients with type Sanders IV bears sural cutaneous nerve injury symptoms,One case of postoperative patients with type Sanders IV bears traumatic arthritis. Conclusion Open reduction and treatment of intraarticular ealcaneal fractures can lead to satisfactory results.
