摘 要:目的 探讨隐匿性肾小球肾炎无症状蛋白尿患者中尿蛋白与足状突细胞标志蛋白(podocalyxin,PCX)、血清胱抑素C(Cystatin C,CST3)的关系,以及诊断预后不良隐匿性肾小球肾炎的应用价值.方法 依据24小时尿蛋白量,将无症状蛋白尿患者(75例)分为四组(〈0.5g/d,0.5~1g/d,1~2g/d,〉2g/d),同时增加无症状蛋白尿史慢性肾衰患者组(23例)及健康对照组(25例).采集研究对象血清与尿液,采用免疫比浊法检测尿足状突细胞标志蛋白、血清胱抑素C. 结果 尿足状突细胞标志蛋白与24小时尿蛋白量呈正相关,随24小时尿蛋白量的升高,尿足状突细胞标志蛋白而升高(P<0.05).胱抑素C与24小时尿蛋白排泄量(〈0.5g/d,0.5~1g/d,1~2g/d)无相关关系,与健康组相比无显著性差异(P〉0.05).但无症状蛋白尿患者(〉2g/d)组,血清胱抑素C与健康组相比有显著性差异(P<0.05). 结论测定尿液足状突细胞标志蛋白和血清胱抑素C有助于预后不良隐匿性肾小球肾炎的早期诊断和预后判断.Objective To investigate the relationship between 24h protein excretion and Podocalyxin, Cystatin C; and the value in the diagnosis of aymptomatic proteinuria with bad outcomes. Methods Aecarding to 24h protein excretion, the aymptomatie proteinuria patients ( 73 cases) were divided into four groups ( 〈 0.5g/d,0.5 - 1g/d, 1 - 2g/d, 〉 2g/d), meanwhile renal failure patients group(23 cases) whh asymptomatie proteinuria and healthy group(25 cases) were available. Collecting the 24h urine and serum, check urinary, podoealyxin, serum Cystatin C. Results podocalyxin have positive relationship with 24b urine protein excretion, increase with the 24h urine protein exeretion(P 〈 0.05). Cystafin C has no difference between asymptomatic proteinuria groups( 〈 0.5 g/d, 0.5 - 1g/d, 1 - 2g/d) ( P 〉 0.05 ), but to the group ( 〉 2g/d), Cystatin C were higher than that in healthy eontrols(P 〈 0.05). Conclusion podoeaylyxin and Cystatin C were very, useful to the early diagnostic and disease forecast of aymptomatie proteinnria with bad outcomes.
