摘 要:目的探讨脑钠素(BNP)对急性心肌梗死预后的评价作用。方法选取我院2009年1--11月收治的32例急性心肌梗死患者作为心肌梗死组,A./E后均在12h内成功再灌注,术后随访6个月,再次发生心血管事件者作为事件组(11例),未发生心血管事件者作为对照组(21例)。另选同期10例排除心肌梗死、心绞痛的其他非心力衰竭患者为非心肌梗死组。测定入院即刻和第1、2、5、14天的血浆BNP水平;心肌梗死患者入院10~14d、第3个月的左室射血分数(LVEF)。结果心肌梗死组患者入院即刻和第1、2、5、14天的血浆BNP水平与非心肌梗死组比较,差异均有统计学意义(P〈0.05);事件组患者的血浆BNP水平在入院即刻和第1、2、5、14天与对照组比较,差异均有统计学意义(P〈0.05)。事件组入院10-14d、第3个月的LVEF值分别为(33±6)%、(46±14)%,对照组分别为(39±11)%、(54±8)%,两组比较差异均有统计学意义(P〈0.05)。结论血浆BNP水平在急性心肌梗死后显著升高,对心肌梗死预后的评价具有重要意义。Objective To explore into the evaluation of brain natriuretic peptide (BNP) in prognosis of acute myocardial infarction (MI) . Methods Totally 32 inpatients with acute myocardial infarction in our hospital from January to November 2009 were enlloed as MI group. These patients were reperfursed sueeeedly in 12 hours after hospitalization, and the postopera- tional follow- up was conducted for 6 months. There were 11 patients who suffered cardiovascular events again (event group), and 21 patients who did not suffer again (control group). And 10 people, who were confirmed not having MI or angina peetoris, were taken as non -MI group. Blood plasma BNP were determined at once, at the lth, 2th, 5th, and 14th day after hospitalization. Left ventricle ejection fraction (LYEF) was determined at the 10 - 14th day after hospitalization and in the 3th month. Results Blood plasma BNP levels of the patients at once, at the 1 th, 2th, 5th, and 14th day after hospitalization in the MI group were higher obviously than those in non - MI group ( P 〈 0. 05 ), and so were in the event group than those in the con- trol group (P 〈0. 05). The value of LVEF at the 10 - 14th day after hospitalization and in the 3th month were (33 ± 6)% and (46 ± 14) % in the event group, and (39± 11 ) % and (54±8 ) % in the control group, the differences between the two groups were statistically significant ( P 〈 0. 05 ) . Conclusion The level of Blood plasma BNP in acute myocardial infarction increases significantly, which is of very important significance for evaluating prognosis of myocardial infarction.
