南京市社区慢性病患者生命质量及影响因素的调查研究

摘　　要：目的评价南京市社区慢性病患者的生命质量,并探讨其影响因素。方法采取随机整群抽样方法,使用社区人群功能状态测定量表(COOP/WONCA),对部分社区的1 457名社区居民进行调查,其中有效样本1 259人,运用多元线性回归分析等对有效调查信息进行分析。结果 1 259名社区居民中,327人未患调查的慢性病,701人患1种慢性病,190人同时患2种慢性病,41人同时患3种及以上慢性病。慢性病患者在情感、日常活动、社会活动、整体健康、疼痛五个维度的生命质量均明显低于不患病人群(P<0.01),不同慢性病患者的生命质量也存在明显差异(P<0.001)。多因素线性回归分析显示,年龄、慢性病、工作情况对生命质量有负面影响,是危险因素;文化程度是保护因素(P<0.05)。结论慢性病患者生命质量较不患病人群低并受多种因素影响,不同类型的慢性病对生命质量均有负面影响。对慢性病患者进行社区干预时,应关注患者的生命质量,并从知识水平、健康行为等方面采取综合干预措施。Objective To evaluate the quality of life （QOL） and investigate the influencing factors of patients with chronic diseases in communities in Nanjing. Methods The cluster random sampling was taken and COOP/WONCA Charts was used to conduct surveys among 1 457 community residents in arts of the communities, of which 1 259 samples were eligible. The results of surveys were treated by using multivariate linear regression analysis. Results Among the 1 259 residents 327 did suffer from the investigated diseases, 701 had one kind of the diseases. 190 had two kinds of the diseases, and 41 had three kinds of the diseases. The patients with chronic diseases had pooler QOL than general people in daily activities, emotion, social activity, hoslistic health, and pain （P 〈0.01）. Results of multivariate linear regression analysis showed that age, chronic diseases, working situation had negative influences on QOL, but better education had positive influence （P 〈 0. 05 ）. Conclusion The patients with chronic diseases had pooler QOL than general people and was subject to a number of factors, different types of chronic diseases have negative influences on QOL. It is necessary to pay attention to the quality of life of patients and resort a complex intervening measure in education and health behavior when we conduct community intervention to the patients with chronic diseases.
