摘 要:目的:探讨FGR孕妇及胎儿脐血清皮质醇浓度的改变与胎盘组织11β-HSD2表达水平间的关系。方法:用放射免疫法测定孕妇及脐血清皮质醇浓度;用免疫组织化学方法结合计算机多媒体彩色病理图像分析技术测定胎盘组织11β-HSD2表达水平;实验所的数据均经SPSS统计软件分析处理。结果:FGR组孕妇及胎儿脐带血清皮质醇浓度和胎盘组织11β-HSD2表达水平明显高于对照组(901.07±365.15vs602.96±237.24;552.75±205.02vs155.40±66.20;101.33±33.48vs135.13±29.951);孕妇血清皮质醇浓度与胎儿脐血清皮质醇浓度呈正相关(r=0.817,P<0.01);孕妇及胎儿脐血清皮质醇浓度与胎儿体重呈负相关(r=-0.388,P<0.05;r=-0.588,P<0.01);胎儿脐血清皮质醇浓度与胎盘组织11β-HSD2表达水平呈负相关(r=-0.385,P<0.05);胎盘组织11β-HSD2表达水平与胎儿体重呈正相关(r=0.649,P<0.01)。结论:母体孕期高浓度的血清皮质醇经胎盘灭活减少,使胎儿血清皮质醇浓度增高导致胎儿宫内发育迟缓。血清皮质醇浓度升高及胎盘组织11β-HSD2表达水平下降是导致FGR的病因之一。Objective Inquire into relationship between change of pregnancy serum cortisol concentration and placental tissue 11β-HSD2 expression level in fetal growth restriction.Method Take advantage of radioimmunoassay(RIA)to assay serum cortisol concentration of pregnancy and fetal umbilical,take advantage of immunohistochemistry(IH)and multimedia colorful patheological image analysis technique to determine placental tissue 11β-HSD2 expression level,all acquired data were dealt with statistic software SPSS.Results Serum cortisol concentration of pregnancies and fetal umbilical and placental tissue 11β-HSD2 expression level in FGR group was manifestly higher than control group〔(901.07±365.15)vs(602.96±237.24);(552.75±205.02)vs(155.40±66.20);(101.33±33.48)vs(135.13±29.951)〕.Serum cortisol concentration of pregnancies positively correlated with fetal umbilical serum cortisol concentration(r=0.817 P0.01);and serum cortisol concentration of pregnancies and fetal umbilical negatively correlated with fetal body weight(r=-0.388,P0.05;r=-0.588,P0.01);and fetal umbilical serum cortisol concentration showed negative correlation with placental tissue 11β-HSD2 expression level(r=-0.385,P0.05);and placental tissue 11β-HSD2 expression level positively correlated with fetal body weight(r=0.649,P0.01).Conclusion High concentration of serum cortisol can go through placenta into fetus body because placenta can not catabolize higher concentration of serum cortisol,at last inhabit fetal intrauterine growth and development.Increased serum cortisol concentration and decreased placental 11β-HSD2 expression level may be one cause of FGR.
