凝固酶阴性葡萄球菌生物膜标志物3种检测方法比较

摘　　要：目的通过对凝固酶阴性葡萄球菌(CNS)在生物膜形成过程中产生的胞间多糖黏附素表型特征和分子标志物检测结果的相关性分析,对刚果红试验、icaD片段PCR扩增及黏附试验进行评价。方法采用定性的刚果红试验、icaD片段PCR扩增试验检测116株临床分离的CNS胞外多糖黏附素产生能力,部分菌株采用定量的黏附试验检测生物膜的形成能力。结果 116株CNS中,刚果红试验阳性率为20.7%,icaD片段PCR扩增阳性率为29.3%,两方法比较,χ2=5.062,P<0.05;刚果红试验和黏附试验结果比较,χ2=5.785,P<0.05;黏附试验结果与icaD片段的PCR扩增结果比较,χ2=0.143,P>0.05。结论生物膜不仅存在于表皮葡萄球菌还存在于其他CNS菌种中。刚果红试验操作简单易行,但敏感性稍差;定量黏附试验操作繁琐,不易质量控制,不适宜常规开展;以PCR技术扩增ica操纵子,方法简单易行,能够满足临床要求。Objective To evaluate the three methods（Congo red test,adherence test and icaD fragment PCR） for the detections of phenotype characteristics of intercellular polysaccharide adhesin and molecular markers produced during biomembrane-forming process of coagulase-negative staphylococci（CNS） by analyzing the correlation of the experimental results.Methods The producing ability of intercellular polysaccharide adhesin was detected by qualitative Congo red test and icaD fragment PCR in 116 CNS strains isolated from clinical patients.The biomembrane formation in a part of the CNS strains was detected by quantitative adhesive test.Results Among 116 CNS strains,the positive rate was 20.7% for Congo red test and 29.3% for icaD fragment PCR,which showed statistical significance by Chi-square test.Significant difference was observed between Congo red test and adhesive test,but no significant difference was found between adhesive test and icaD fragment PCR.Conclusions Biomembrane formation was observed in either S.epidermidis or other CNS strains.Congo red test was easy to be performed,but its sensitivity was lower.Quantitative adhesive test was not available for the routine laboratory because of the difficult quality control.The PCR amplifying ica operon was simple and easy for clinical application.
