血清脂联素水平与冠心病关系的研究

摘　　要：目的检测血清脂联素(APN)水平,探讨其在CHD发病中的作用。方法收集150例冠状动脉造影阳性的CHD患者和104例体检健康者血清,用ELISA法检测APN和apo A5水平,7600-020E全自动生化分析仪检测TC、TG、LDL-C、HDL-C、Lp(a)等。结果与健康对照组比较,CHD组患者的HDL-C水平显著降低,LDL-C、TG、TC、Lp(a)均增高。稳定型心绞痛(SAP)患者和急性冠脉综合征(ACS)患者血清APN浓度均明显低于健康对照组(P<0.05),ACS患者血清APN浓度较SAP患者进一步降低(P<0.05);SAP患者血清apo A5浓度为(209.8±52.6)ng/ml,显著低于健康对照组(231.5±57.0)ng/ml,P<0.05。ACS组血清apo A5浓度[(248.6±54.4)ng/ml]显著高于SAP组[(209.8±52.6)ng/ml],P<0.01。Pearson相关分析显示经年龄、性别校正后APN水平与LDL-C、TG呈负相关,与HDL-C呈正相关;SAP组APN与apo A5呈正相关(P<0.05)。结论低水平血清APN和异常升高apo A5不仅与CHD类型密切相关,还可作为CHD患者预后评价的一个新指标。Objective To measure adiponectin（APN） level in serum of coronary heart disease（CHD） patients and evaluate the association of APN with CHD.Methods The serum samples of 150 CHD patients confirmed by coronary angiography and 104 healthy subjects for physical examinations were collected.The levels of APN and apolipoprotein A5（apo A5） in serum were measured by ELISA.Total cholesterol（TC）,triglycerin（TG）,low density lipoprotein cholesterol（LDL-C）,high density lipoprotein cholesterol（HDL-C） and Lp（a） were measured by 7600-020E automatic biochemistry analyzer.Results Compared to the healthy control group,the level of HDL in CHD group was apparently lower,and the levels of LDL-C,TG,Lp（a） were higher.The levels of serum APN in the patients with stable angina pectoris patients（SAP） and acute coronary syndrome（ACS） were significantly lower than those in control subjects（P 0.05）,and APN level of ACS patients was lower than that of SAP（P 0.05）.The concentrations of apo A5 [（209.8 ± 52.6） ng/ml]in the serum of SAP patients were lower than those of healthy controls [（231.5 ± 57.0） ng/ml]（P 0.05）.The concentrations of apo A5 in ACS serum [（248.6 ±54.4） ng/ml]were lower than those in SAP serum [（209.8 ± 52.6 ng/ml） ]（P 0.01）.Pearson correlation analysis showed that APN level after adjustment for age and sex was significantly negatively correlated with LDL-C and TG,but positively correlated with HDL-C.APN level was positively correlated with apo A5 in SAP serum.Conclusions Lower APN and higher apo A5 may be correlated with CHD of certain types and used as a new markers for prognostic evaluation of CHD patients.
