摘 要:目的 探讨低温射频等离子(temperature-controlled radio-frequency, TCRF)辅助下改良悬雍垂腭咽成形术(H-UPPP)治疗重度阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea hyopnea syndrome,OSAHS)的疗效.方法 全身麻醉下对25例重度OSAHS行TCRF辅助下H-UPPP,术后6个月复查多导睡眠监测(PSG),判定疗效.结果 治愈5例,显效15例,有效3例,无效2例,总有效率92%.术后6个月睡眠呼吸暂停低通气指数(AHI)和最低血氧饱和度(L-SaO2)与术前比较明显改善:AHI P〈0.01,最低SaO2 P〈0.05.结论 低温等离子射频辅助下H-UPPP是治疗多平面阻塞、重度OSAHS的一种有效方法.Objective To explore the therapeutic efficacy of severe obstructive sleep apnea hyopnea syndrome (OSAHS) by temperature controlled radio - frequency (TCRF) Aided H - uvulopalatopharyngoplasty ( H - UPPP). Methods 25 cases of severe OSAHS diagnosed by polysomnography (PSG) and by Muller's maneuver were treated surgically by H -UPPP combined with TCRF. All the cases received polysomnography before the operation and 6 month after the operation. Results For 25 cases,5 patients were cured, 15 patients were improved,3 patients were effective and 2 patients were not improved. The total effective rate was 92%. AHI and L - SaO2 were improved significantly compared with those before operation. Conclusion TCRF Aided H - UPPP is one of the effective methods to treat severe OSAHS.
