摘　　要：目的探讨Syndecan-1在大肠癌组织中的表达及其与临床各病理因素的关系及临床意义。方法采用免疫组化SP法测定58例大肠癌、12例大肠腺瘤和9例正常结肠组织Syndecan-1的表达水平。结果正常大肠粘膜组织中Syndecan-1呈阳性-强阳性表达,在大肠癌组织中的表达则显著减弱甚至缺失。大肠癌组织中Syndecan-1的表达较其在腺癌和正常大肠组织中的表达有显著的降低(P=0.024,P=0.001)。Syndecan-1在大肠癌不同细胞病理分化程度间的表达有显著性差异,低分化者的表达明显弱于高分化及中分化者(P=0.003,P=0.001)。在有远处转移的大肠癌组织中Syndecan-1的表达比无转移的大肠癌明显减弱(P=2.23,P=0.03)。Syndecan-1阳性患者的预后明显优于Syn-decan-1阴性患者(P=0.0046)。Cox比例风险模型多因素分析结果表明Syndecan-1的表达是判断患者预后的独立因素。结论 Syndecan-1的低表达与结直肠癌细胞的转移潜力密切相关,而且是大肠癌患者预后不良的独立的危险因素。Objective To evaluate changes in Syndecan-1 expression during the development of human colorectal cancer.Correlation between tumour expression of Syndecan-1 and clinicopathological characteristics of patients with colorectal carcioma was studied.Methods 58 human colorectal cancer patients,12 of colorectal adenoma and 9 normal colorectal epithelium were studied,the expression of Syndecan-1 were examined by immunohistochemical method.Results Histologically,normal mucosa shows strong membranous and cytoplasmic staining of Syndecan-1,while the poorly differentiated adenocarcinoma shows a significant reduction in the expression of Syndecan-1 than that of well and moderately differentiated adenocarcinoma（P=0.003,P=0.001）.Clinically,the inidence of metastasis to distant place in patients with Syndecan-1-negative tumors was significantly high（t=2.23,P=0.03）.When analyzed for prognostic significance,patients with Syndecan-1-positive cancer showed a better prognosis than those with Syndecan-1-negative cancer（P=0.0104）,and a multivariate Cox proportional hazard model identified reduced Syndecan-1 expression as an independent predictive factor for overall survival（P=0.001）.Conclusion Our results suggest that the reduction of Syndecan-1 expression in colorectal cancer cell is closely related to the invasive character of colorectal cancer.The evaluation of Syndecan-1 expression in colorectal cancer may allow prediction of patients′survival after surgery.
