摘  要:目的探讨经外侧裂-岛叶入路显微手术治疗高血压基底节区脑出血的手术方法和疗效。方法自2007年1月~2010年1月对32例高血压基底节脑出血采取经外侧裂-岛叶入路显微手术清除血肿。结果术后1d复查头部CT,血肿清除>80%26例,60%~80%6例,术后6个月根据GOS评分恢复良好9例(28.1%),中残12例(37.5%),重残4例(12.5%),植物生存1例(3%),死亡6例(18.7%)。结论经外侧裂-岛叶入路显微手术治疗基底节区脑出血是一种损伤小、疗效好、预后客观的微创手术方法。Objective To study the therapeutic effects for cerebral in basal ganglia by microneurosurgery via transsylvian fissure-insular approach.MethodsRetrospective analysis was performed in 32 cases with cerebral hematomas in basal ganglia region which were treated by microsurgery via transsylvian fissure-insular approach in our department from 2007.1 to 2010.1.ResultsWith 6 months follow-up study after surgery and according to Glasgow Outcome Score(GOS),the cases of good recovery(GR),moderate disability(MD),severe disability(SD),persistent vegetative state(PVS) and dead(D) were 9 cases(28.1%),12 cases(37.5%),4 cases(12.5%),1 cases(3%) and 6 cases(18.7%) respectively.Conclusion Micronneurosurgical treatment via transsylvian fissure-insular approach for cerebral hematomas in basal ganglia region is a kind of minimally invasive method with less damage,better effects and more favorable prognosis.
