摘 要:目的:观察脊髓受压大鼠在不同时期减压后诱发电位、运动功能评分及脊髓组织病理变化。方法:45只SD大鼠随机分为A、B、C3组,制作脊髓损伤受压模型。3组分别于5h、3d、8d行减压术。各组造模前后及减压术后即刻检测SEP、MEP及运动功能评分。各减压时段后5d均检测SEP、MEP及运动功能评分,并取脊髓标本,行组织病理光镜检查。结果:随着脊髓压迫时间的延长,SEP、MEP潜伏期逐渐延长,波幅逐渐减小,波幅变化较潜伏期更为敏感。恢复过程中,脊髓受压时间越短,诱发电位恢复越早。潜伏期恢复早于波幅,而且SEP恢复早于MEP,MEP恢复早于功能评分。结论:SEP、MEP对脊髓损伤十分敏感,能较早反应脊髓损伤程度;大鼠脊髓损伤后,早期手术减压对损伤的大鼠轴索有保护作用,能减少脊髓损伤面积,促进大鼠后肢功能恢复。Objectives:This study was aimed to observe the pathological changes in evoked potential, motor function score and spinal cord tissue in compressed rat spinal cord at different stages of decompression. Methods:Forty-five SD rats were divided into three groups (A, B, and C) to establish models of compressed spinal cord injury. Three groups were operatively decompressed 5 hours, 3 days, and 8 days after modeling, respectively. SEP, MEP and motor function score were tested before and after rat models were formed and immediately after the operative decompression. The above procedure was repeated 5 days later and spinal cord was sampled to conduct histopathological light microscope test. Results: With the increase in the timing of spinal cord compression, SEP and MEP delitescence gradually grew while the amplitude decreased. Change in amplitude was more sensitive than that of the delitescence. During the period of restoration, the shorter time the spinal cord was compressed, the earlier the evoked potential recovered. The recovery of delitescence was earlier than that of the amplitude; SEP was earlier than SEP and MEP was earlier than function score.Conclusion:Sensitive to spinal cord injury, SEP and MEP are able to reflect the injury of spinal cord quite early; after the rat spinal cord is injured, immediate operative decompression protects the axonal injury in rats, decreases the lesion area of injured spinal cord and accelerates the recovery of the function of rat hind limb.
