摘 要:目的对比观察高血压、糖尿病对内皮功能和动脉弹性的不同作用。方法选取高血压合并糖尿患者50例(HP+DM组),单纯高血压患者46例(HP组),单纯糖尿病患者48例(DM组)以及健康志愿者50例(NT组),采用美国HDI公司CV ProfilorTM DO2020型动脉弹性功能测定仪系统检测大动脉弹性指数(C1)以及小动脉弹性指数(C2),并采用流式细胞术检测受试者血浆中CD31+/CD42-内皮细胞微颗粒的水平。结果与DM组及NT组相比,HP+DM组与HP组的C1、C2显著降低;与HP、DM及NT组相比,HP+DM组的内皮微颗粒水平显著升高,DM组的内皮微颗粒水平比HP组显著升高;C1、C2与脉压、餐后2h血糖呈负相关;内皮微颗粒水平与脉压及餐后2h血糖呈正相关,与空腹血糖无明显的相关性。结论高血压在影响动脉弹性方面起重要作用,且主要表现在对大动脉弹性的影响;糖尿病主要影响小动脉弹性,会导致内皮微颗粒释放显著增加。Objective To study the influences of hypertension and diabetes on arterial compliances and endothelial microparticle level.Methods Fifty patients with hypertension and diabetes,46 with hypertension,and 48 with diabetes were enrolled in this study,with 50 healthy volunteers group serving as the normal control.Large arterial compliance(C1) and small arterial compliance(C2) were measured using CVProfilorTM DO2020 cardiovascular profiling system,and the levels of CD31+/CD42-endothelial microparticles(EMPs) were determined by flow cytometry in these subjects.Results Compared with the hypertensive patients,diabetic patients and healthy volunteers,the patients with both hypertension and diabetes showed significantly decreased C1 and C2.C1 and C2 were significantly decreased in the hypertensive patients in comparison with those in the diabetic patients and the healthy volunteers.Compared with the healthy volunteers group,the diabetic patients showed markedly decreased C2 but normal C1.The levels of CD31+/CD42-EMPs were significantly increased in patients with both hypertension and diabetes compared with the othergroups,and the diabetic patients had higherEMPs levels than the hypertensive patients.Pearson regression analysis indicated that C1 and C2 were inversely,while the levels of EMPs positively,correlated to the pulse pressure and 2 h plasma glucose;the levels of EMPs were not related with fasting glycemia.Conclusions Hypertension appears to have greaterimpact on arterial compliance,especially C1,while diabetes is the major factorresponsible forincreased release of EMPs.
