摘 要:目的探讨采用药物干预老年糖尿病餐后高三酰甘油(TG)患者血管内皮功能的改变。方法选取经脂肪餐试验确立的单纯餐后高TG的老年糖尿病患者21例,予以苯扎贝特治疗3个月,观察TG以及一氧化氮(NO)、内皮素-1(ET-1)、纤溶酶原激活物抑制剂-1(PAI-1)、组织型纤溶酶原激活物(t-PA)等血管活性因子和颈动脉内中膜厚度(IMTc)、血管内皮依赖性舒张功能(FMD)的改变。结果与治疗前相比,苯扎贝特治疗后患者脂肪餐试验的血清TG水平显著降低,同时NO/ET-1显著升高,PAI-1/t-PA显著降低;而且IMTc和△FMD显著下降(P<0.05或0.01)。结论苯扎贝特可降低老年糖尿病患者餐后高TG血症,还可改善异常的血管内皮功能。Objective To observe the change of vascular endothelial function in elderly diabetic patients with postprandial hypertriglyceridemia after drug intervention and provide the clinical evidence for early reducing hypertriglyceridemia and preventing the development of atherosclerosis (AS).Methods 21 elderly diabetic patients with single postprandial hypertriglyceridemia were selected by oral fatty meal test(TG4 h2.0 mmol/L) and treated with Bezafibrate for 3 months.Changes of serum triglyceride (TG) level and vascular endothelial activated factors such as nitric oxide (NO),endothelin-l (ET-1),plasminogen activator inhibitor-1 (PAI-1) and tissue plasminogen activator (t-PA) were detected.Meanwhile,the effect on carotic intima-media thickness (IMTc) and the flow-mediated vasodilation (FMD) was observed.Results Serum TG level of the patients was lower,the ratio of NO/ ET-1 was higher and PAI-1/t-PA was lower significantly after drug therapy,and the levels of IMTc and △FMD were significantly lower also (P0.05 or P0.01).Conclusions Bezafibrate can reduce postprandial hypertriglyceridemia and improve abnormal vascular endothelial function in elderly diabetic patients.
