孕期沙眼衣原体、支原体感染治疗效果与胎儿状况相关性研究

摘　　要：目的：研究孕期沙眼衣原体、支原体感染治疗效果与胎儿生长发育的关系。方法：应用免疫荧光法对６００例孕妇宫颈分泌物行沙眼衣原体、支原体属抗原检测，阳性者对其配偶行尿道分泌物检测。孕妇阳性者给以口服红霉素治疗，观察治疗过程中，胎儿异常及低体重儿发生比较及与配偶感染的关系。结果：孕期衣原体及混合感染，红霉素治愈组胎儿异常（流产、死胎、畸形、新生儿死亡）及低体重儿与持续感染状态组比较，差异显著（Ｐ＜０．０５）。支原体治愈组胎儿异常发生率３．４５％（１／２９），无低体重儿，而未治疗组胎儿异常１３．１％（１１／８４），低体重儿７．１４％（６／８４）两组虽有明显差别，但无统计学意义。配偶感染孕妇治疗效果差。结论：孕期沙眼衣原体、支原体感染红霉素治疗后可减少胎儿异常及低体重儿发生率，孕妇感染治疗效果与配偶感染，关系密切。Objective:To find out the relationship between the therapeutic effects on tracomatis or mycoplasma infection during pregnancy and fetal growth. Methods: Cervical discharge from 600 pregnant women and the urethra secretion from the partners of those who were positive with C.trachomatis and Mycoplasma antigens by immunoflurescent method. The oral therapeutical doses of erythromycin given to the positive women and the occurrence of fetal abnormality, low weight birth, etc, were observed. And the relationships to whether their partners in fected were analyzed. Results: As compared with the untreated C.tracomatis infected group, rates of fetal abnormalities (abortion, stillbirth, deformity and new born death) and low weight births in the erythromycin treated group were much lower ( P <0 05). In the erythromycin treated myeoplasma infected group, fetal abnormality rate was 13.1% (11/89) and low weight birgh rate 7.14%, but the difference was of no statistic significance. The curative effects on those women whose parthers were infected were poor. Conclusion: Erythromycin therapy may help reducing the occurrence of fetal abnormalities and low weight birth due to intrauterine C.trachomatis and mycoplasma infections during pregnancy. And the curative effects on the infected pregnant women are highhy related to whether their partners infected or not.
