摘 要:目的 探讨过氧化脂质(LPO)和红细胞超氧化物歧化酶(RBCSOD) 活性在妊娠肝内胆汁瘀积症(ICP)病理生理变化中的作用。 方法 分别用硫代巴比妥酸荧光测定法和邻苯三酚自氧法测定同期正常晚期妊娠妇女20 例(对照组) 及妊娠肝内胆汁瘀积症(ICP) 患者40 例(ICP组) 的血浆及脐静脉血LPO 和RBCSOD 活性。 结果 (1)ICP 组血浆LPO、RBCSOD 活性和LPO/RBCSOD比值均明显高于对照组( 分别为P< 0.01,P< 0 .01 ,P< 0.05) 。(2)ICP 组脐静脉血LPO 和LPO/RBCSOD比值均明显高于对照组( P< 0.01) ,RBCSOD 活性虽高于对照组,但无显著性差异(P> 0.05) 。(3)ICP组平均孕周和新生儿体重明显短于和低于对照组( 分别为P< 0.05 ,P< 0.01),ICP组羊水胎粪污染率明显高于对照组( P< 0.01) 。(4)ICP 组脐静脉血LPO 和LPO/RBCSOD 比值均与新生儿体重呈明显负相关( 分别为r= -0 .683, P< 0.01 及r= - 0.623 , P<0 .01)。 结论 ICP时胎儿胎盘内氧化与抗氧化平衡严重失调所致的胎?Objective To evaluate the role of lipid peroxide (LPO) and superoxide dismutase in red blood cell(RBC SOD) in the pathophysiological changes of intrahepatic cholestasis during pregnancy(ICP). Methods 40 patients with ICP were chosen as an observation group,20 healthy women in their late pregnancy were served as a control group. Maternal plasma and umbilical cord vein concentration of LPO and the amount of RBC SOD were measured by thiobarbituric acid fluorimetric determination and catechol self oxidation determination respectively. Results (1)The plasma levels of LPO 、RBC SOD and LPO/RBC SOD ratio in the ICP group were significantly higher than those in the control group ( P<0.01, P<0.01, P <0.05). (2)The levels of umbilical cord vein LPO and LPO/RBC SOD ratio in the ICP group were significantly higher than those in the control group ( P<0.01,P <0.01). The level of RBC SOD of umbilical cord vein in the ICP group was higher than that in the control group, but it was not significant ( P >0.05). (3)The average gestational age in the ICP group was significantly shorter than that in the control group ( P <0.05) and the weight of newbron was also significantly lower ( P <0.01).The rate of meconium stained amniotic fluid in the ICP group was significantly higher than that in the control group ( P <0.01).(4)There were a definite negative correlation between LPO, LPO/RBC SOD ratio of umbilical cord vein and the weight of newborn in the ICP group ( r=-0.683,P<0.01; r=-0.623,P <0.01 respectively). Conclusion The serious imbalance between oxidation and antioxidation in fetal placental unit in ICP causes placental injury and may be closely related to the adverse perinatal outcomes.
