摘　　要：目的探讨他汀类药物在短暂性脑缺血发作(transient ischemic attack,TIA)二级预防应用中依从性的影响因素及护理对策。方法根据依从性判断标准,对240例TIA患者他汀类药物依从性结果进行判断,比较患者性别、年龄、文化程度、经济收入、TIA病程、TIA相关知识、他汀类药物知识及应用时间、他汀类药物不良反应、医护关系、治疗费用等变量对其治疗依从性的影响。选择单因素分析显示有统计学意义的因素赋值后进行多因素非条件Logistic回归分析,筛选出危险因素。结果 240例患者中依从性好者110例,依从性差者130例;单因素分析10个变量有统计学意义(P<0.05),Logistic多元回归分析筛选出4个危险因素:高龄、他汀类药物知识缺乏、护患关系一般、发生药物不良反应。结论 TIA患者服药依从性影响因素有高龄、他汀类药物知识缺乏、护患关系一般、发生药物不良反应,应加强老年患者服药管理,通过多种途径宣传他汀类药物应用知识,建立良好护患关系,预防他汀类药物的不良反应以提高TIA患者服药依从性。Objective To explore factors influencing statins compliance in secondary prevention and its nursing strategy for patients with transient ischemic attack（TIA）.Methods The statins compliance of 240 patients were analyzed with the standard of compli-ance in term of gender,age,educational background,income,course of TIA,related knowledge of TIA,application and adverse reac-tion of statins and its knowledge,relationships between patients and nurses,treatment cost.Multivariate non-conditional Logistic re-gression analysis was applied for factors with statistical significance so as to obtain information about risk factors.Results There were 110 patients with good compliance while the rest were with relatively weak compliance.Single factor analysis indicated that ten variables were related factors influencing statins compliance（P〈0.05）,and multivariate Logistic regression analysis provided four risk factors including the advanced age,lack knowledge about statins,poor relationships between patients and nurses,adverse drug reac-tion.Conclusion Factors that influence the drug compliance of patients with TIA are the advanced age,lack knowledge about statins,poor relationships between patients and nurses,adverse drug reaction.Drug management including knowledge about statins,the prevention of adverse drug reaction for elderly patients should be strengthened.Good relationships between patients and nurses can improve the compliance.
