老年鼻部疾患258例临床特点分析

摘　　要：目的总结老年鼻部疾患的临床特点,以提高老年鼻部疾病患者围治疗期安全性。方法回顾分析2007至2009年老年鼻部疾患住院患者258例的特点及治疗方法。结果 258例老年鼻部疾患中,鼻腔鼻窦炎性患者120例(46.5%),鼻出血患者69例(26.7%),鼻部肿瘤患者64例(24.8%);约221例(85.6%)患者采取了鼻内镜相关的低危治疗或手术。158例(61.2%)伴发老年合并症,主要老年合并症为心血管系统合并症(占63.3%)及糖尿病(占29.1%),且往往多合并症并存,103例(65.2%)伴发2种及以上合并症。老年合并症发病率性别间和不同疾病组间差异无统计学意义。2例延期手术,9例选择非手术治疗。发生围治疗期并发症3例。结论老年鼻部疾患患者有其独特的病理生理特点,应重视对其并存症的诊治,尽量减轻手术创伤,可有效减少手术风险及术后并发症。Objective To summarize the clinical features of nasal diseases in elderly patients so as to make the peri-op- erative treatment safe. Methods The clinical data of 258 elderly inpatients suffering from nasal diseases during 2007 to 2009 were retrospectively analyzed. Results In this cohort, 120（46.5% ） patients suffered from nasal inflammation, 69 （26.7%）of them suffered from epistaxis, and 64（24.8% ） from various tumors. 221 （85.6%） were subjected to endoscopic therapy which belongs to the low danger category. It was found that 158（61.2% ） of the patients were com- plicated with chronic diseases, in which the major complications for the elderly were cadiovascular system diesease and diabetes. There was no statistic significance between gender and different disease groups. Two operations were post- poned, and 9 were subjected to non-operative methods. Peri-operative complications happened in 3 of the elderly pa- tients. Conclusions Geriatric patients have their own specific pathophysiologics. In particular, more attention should be paid to the diagnosis and treatment of co-existent diseases and try to reduce surgical trauma in these elderly patients, which, in turn, can effectively minimize surgical risk and postoperative complications.
