针刺配合穴位注射复方当归注射液治疗胃癌晚期疼痛的初步观察

摘　　要：为了探讨针刺配合穴位注射复方当归注射液治疗胃癌晚期疼痛的疗效,将60例晚期胃癌中、重度疼痛患者随机分2组,对照组30例,口服硫酸吗啡控释片;治疗组30例,采用先针刺后郄穴穴位注射的方法。结果提示,临床总有效率两组差异无统计学意义,χ2=0.02,P=0.990;疼痛视觉模拟评分(VAS)治疗组与对照组治疗前后差异均有统计学意义(P<0.05),治疗后组间比较差异均无统计学意义,P>0.05。但镇痛起效时间两组差异有统计学意义,P<0.05。治疗组不良反应发生率为83.33%,对照组为26.67%,差异有统计学意义,χ2=29.39,P<0.0001。初步研究结果提示,针刺、穴位注射合用与口服硫酸吗啡控释片都可缓解胃癌晚期中重度疼痛,而前者镇痛起效快,同时避免吗啡类药物的毒副反应及成瘾性等不良反应。The objective of to observe the curative effect on the pain of advanced stomach carcinoma with acupuncture and therapy of point injection.A total of 60 advanced stomach carcinoma patients with medium extent pain and severe pain were divided into two groups randomly.A total of 30 patients in control group used Morphine Sulfate Controlled-release Tablets（MS.Contin）.A total of 30 patients in experiment group were firstly acupunctured,then got therapy of point injection in xi-point.As the results,the data of experiment group had no difference with that of control group in clinical total effective frank （χ2=0.02,P=0.990）.In VAS evaluation,there was significant difference between the data before and after treatment in both groups （P0.05）.There was no difference between the data after treatment of the two groups （P0.05）.But the onset time for relieving pain of experiment group was different from that of control group （P0.05）.And the bad reaction rate in experiment group was 83.33%,while that in control group was 26.67%.There was very significant difference in the two groups （χ2=29.39,P0.000 1）.In conclusions,the fundamental research result shows that the method of acupuncture allied with therapy of point injection had the same curative effect as the method of taking Morphine Sulfate Controlled-release Tablets in relieving pain of advanced stomach carcinoma.But the former method had curative effect with shorter time and had fewer bad reaction.
