摘  要:背景前期研究表明冠状动脉粥样硬化性心脏病(冠心病)患者外周血髓样树突状细胞(mDCs)比例较正常人明显下降。目的探讨冠心病患者外周血树突状细胞(DCs)亚型分布与髓过氧化物酶(MPO)浓度变化的关系,为DCs迁徙入动脉粥样硬化(AS)斑块内诱导斑块不稳定提供依据。方法连续入选2008-11-2009-12在南方医科大学珠江医院心内科住院经冠状动脉造影术检查及治疗者88例:造影检查正常对照28例,稳定型心绞痛10例,不稳定型心绞痛25例,急性心肌梗死25例。采用流式细胞仪四色荧光标记技术对mDCs和浆细胞样树突状细胞(pDCs)占外周血单核细胞比例进行检测,酶联免疫吸附测定法检测外周血浆MPO浓度,评价各组间差异,并对外周血mDCs与外周血浆MPO进行相关性分析。结果急性心肌梗死和不稳定型心绞痛组患者mDCs占外周血单核细胞比例及mDCs实际数量明显低于对照组和稳定型心绞痛组[mDCs比例:(0.53±0.27)%、(0.49±0.26)%比(1.26±0.35)%、(1.09±0.42)%;mDCs数量:(48.26±23.14)、(41.63±27.79)比(85.87±32.49)、(78.29±32.65)/μL;均P<0.01];对照组、稳定型心绞痛、不稳定型心绞痛和急性心肌梗死组MPO浓度分别为(0.19±0.15)、(0.45±0.21)、(0.48±0.19)和(0.37±0.16)ng/L,与对照组比较,冠心病患者外周血MPO浓度均明显升高(均P<0.01)。急性冠脉综合征患者血浆MPO浓度与外周血mDCs比例呈负相关(r=-0.881,P<0.01)。结论急性冠脉综合征患者外周血mDCs比例明显降低与不稳定斑块的发生、发展密切相关。Background Preliminary study on peripheral blood dendritic cell subtype distribution showed that the ratio and number of myeloid dendritic cells(mDCs) declined in patients with coronary heart disease compared to healthy controls.Objective To assess the correlation between the peripheral blood dendritic cell subtype distribution and the plasma myeloperoxidase (MPO) concentration in patients with coronary heart disease; to provide the evidence for DCs migrating into the artery atherosclerosis (AS) plaques,mediating local immune plaque inflammation,and inducing plaque instability.Methods Eighty-eight patients were enrolled after routine and coronary angiography examinations between november 2008 and december 2009.The proportion of peripheral blood mDCs and plasmacytoid dendritic cells (pDCs) accounted for peripheral blood mononuclear cells were detected by flow cytometry using four-color fluorescent markers; and plasma MPO was detected by enzyme-linked immunosorbent assay.Correlation between the peripheral blood myeloid dendritic cells and plasma MPO was analyzed.Results The proportion and absolute number of peripheral blood mDCs in acute myocardial infarction (AMI) group and unstable angina pectoris(UAP) group were significantly lower than those in control and stable angina pectoris(SAP) group [mDCs proportion:(0.53±0.27)%,(0.49±0.26)% vs (1.26±0.35)%,(1.09±0.42)%; quantity:(48.26±23.14),(41.63±27.79) vs (85.87±32.49),(78.29±32.65) /μL; all P〈0.01].The plasm MPO concentrations in control,SAP,UAP and AMI group were (0.19±0.15),(0.45±0.21),(0.48±0.19) and (0.37±0.16)ng/L,which were significantly higher in patients with coronary heart disease than in control group (all P〈0.01),and were negatively correlated with the blood mDCs in patients with acute coronary syndrome(r=-0.881,P〈0.01).Conclusion The decreased circualting mDCs percentage is closely related to the formation and development of unstable plaque in pati
