摘 要:目的探讨正常血压人群血压变化与钠钾反应性的关系。方法 2004-04-10对陕西眉县正常血压成人(年龄>16岁)339位进行低盐饮食7d,高盐饮食7d的慢性盐负荷实验,之后进行高盐+补钾干预7d,以平均动脉压(MAP)升高≥10%为盐敏感组(n=80),<10%为盐不敏感组(n=259);以补钾后MAP下降10%为标准分为补钾MAP下降≥10%组(n=61),补钾MAP下降<10%组(n=278)。于2009年对该队列人群进行随访调查。结果①完成随访共275人,总随访率为81.2%,其中盐敏感组随访率为83.8%,盐不敏感组为80.3%,补钾MAP下降≥10%组为83.6%,补钾血压下降<10%组为80.6%。②5年后,与盐不敏感组比较,盐敏感组血压变化幅度大,无论收缩压还是舒张压均明显升高,高血压患病率也显著升高。③与补钾MAP下降<10%组比较,补钾MAP下降≥10%组的血压变化幅度较小。结论盐敏感性是高血压发病的重要危险因素,血压对钾敏感是高血压的一个保护因素。Objective To investigate the founction of changing blood pressure due to sodium and potassium intake on assessing blood pressure,and its trending to develop to hypertension in normotensive adults.Methods A baseline survey was carried out in 339 normotensive adults in 2004.All subjects were maintained on a protocol with 7 days low salt diet,7 days high salt diet,and then high salt diet with potassium supplementation for another 7 days.Salt sensitivity (SS) were found in 80 subjects,non-salt sensitivity 259 in subjects; Based on the responses of blood pressure to potassium intake after 7 days high salt diet with potassium supplementation,all subjects were divided to the group with decreasing mean artery pressure (MAP) equal or greater than 10% or the group with decreasing MAP less than 10%.The cohort was revisited 5 years later.Results The total returning response rate of the cohort was 81.2%,while 83.8% for SS,and 80.3% for NSS,83.6%for the group of MAP≥10% to potassium intake and 80.6% for MAP10% to potassium intake.The increments in both SBP and DBP in SS group were greater than NSS group.The incidence of hypertension in SS group was also higher than that in NSS.In contrast,the variations of blood pressure in the group of MAP≥10% to potassium intake were smaller than the group of MAP 10% to potassium intake.Conclusion Salt sensitivity is an important risk factor for development of hypertension.However,the sensitivity of blood pressure to potassium intake may be a protective factor for high blood pressure.
