摘  要:目的探讨肝脏大血管对HIFU治疗能量沉积的影响。方法采用磁共振导航的HIFU治疗系统,通过温度图实时监控,辐照距肝门静脉不同距离正常山羊肝组织。结果焦点距血管壁0、5mm及10mm时,焦点处温度升高达(66.8±2.7)℃,(67.3±2.6)℃和(66.5±2.5)℃时所需平均能量分别为(3111.0±398.4)J、(2780.9±351.3)J及(2411.0±281.4)J;平均升高1℃所需能量分别为(46.6±6.5)J、(41.4±5.5)J及(36.3±4.2)J;而肝组织凝固性坏死体积无差异;辐照后血管无异常。结论 HIFU能安全有效的消融大血管旁肝组织,越靠近大血管,所需要的能量越高。Objective To investigate the effect portal vein(PV) on high intensity focused ultrasound energy deposition in liver tissue. Methods Normal goat liver tissue from different distances to the main branches of PV was ablated by magnetic resonance imaging-guided high-intensity focused ultrasound. Results When the focus was at the distance of 0,5 and 10 mm to the PV wall,the ultrasound energy needed to increase the temperature to (66.8±2.7) ℃, (67.3±2.6) ℃ and (66.5±2.5) ℃ was (3 111.0±398.4) J,(2 780.9±351, 3) J and (2 411±281.4) J,and the energy for 1 ℃ was (46.6±6.5) J,(41.4±5.5) J and(36.3±4.2) J,respectively. There was no statistic difference for coagulative necrosis volume and no obvious changes for vessels. Conclusions HIFU can ablate liver tissue close to large blood vessel safely and effectively. The closer to the vessel the loci was, the more energy was needed.
