摘　　要：目的应用实时三维超声(RT-3DE)技术分节段评估心肌梗死(MI)后左室梗死和非梗死节段心肌功能的改变,探讨RT-3DE技术在评估缺血性心肌病左室功能方面的应用价值。方法将40例MI患者与40例正常人按照随机化原则一一配对。根据美国心脏协会(AHA)17节段法将40例患者心脏节段分为A组梗死节段组和B组非梗死节段组,40例正常人心脏对应MI患者A组和B组相应节段位置和数目分为a、b2个对照组。运用RT-3DE获取A、B及其对照组节段舒张末期容积(rEDV)、节段收缩末期容积(rESV)、节段射血分数(rEF)、节段-整体射血分数(rgEF)和节段心搏量与左室心搏量的比值(rgSVRatio),并进行比较。结果 A组、B组rESV、rEDV均较正常对照组增大,EF及A组、B组rEF、rgEF均减低(P<0.01);A组rgSVRatio较正常对照组下降、B组rgSVRatio增大(P<0.01)。结论 RT-3DE技术可以更准确地分节段评估心肌梗死后左室功能的改变。Objective To assess the change of systolic function in infarcted and non-infarcted regions of left ventricular （LV） in patients with myocardial infarction （MI） by real-time three-dimensional echocardiography （RT-3DE）. Methods 40 patients with MI and 40 normal subjects were chosen for the study and they were paired by the random one-to-one match rule. Then according to 17 sub-division method of American Heart Association,all cardiac segments with MI were divided into group A with regions of infarction and group B with non-infarcted regions. The cardiac segments of 40 normal subjects were divided into two corresponding groups according to group A and B. RT-3DE was performed and a series of indicators were plotted. Then the regional end-diastolic volumes （rEDV）, regional end-systolic volumes（rESV） ,regional ejections fractions（rEF）, regional stroke volume to global diastolic volume（rgEF） and ratio between regional and global stroke volume（rgSVRatio）were compared respectively between group A,B and the control group. Results rESV and rEDV of group A and B were larger than those from the control group, rEF and rgEF de- creased（P^0.01）. The rgSVRatio of group A was lower than that from the control group,but group B increased（P~ 0.01）. Conclusions RT-3DE can precisely assess the global and regional geometrical,volume and functional changes simultaneously in MI patients.
