右室Tei指数对伴肺动脉高压房间隔缺损患者右心室功能的研究

摘　　要：目的 (1)运用组织多普勒测量右心室Tei指数评价伴肺动脉高压的房间隔缺损(atrial septal defect,ASD)患者右心室功能;(2)通过右心室Tei指数研究房间隔缺损患者成功施行修补术后右心室功能的改善情况。方法伴有不同程度肺动脉高压的房间隔缺损患者28例,正常人33例。运用组织多普勒方法计算右室Tei指数。通过三尖瓣反流估测房间隔缺损患者的肺动脉压力。结果 (1)伴有肺动脉高压的房间隔缺损患者右心室Tei指数明显大于正常对照组。(2)房间隔缺损患者修补术后1周右心室Tei指数及肺动脉压力较术前明显减小。结论 (1)伴肺动脉高压的房间隔缺损患者存在不同程度的右室功能降低。(2)房间隔缺损患者成功施行修补术后肺动脉压力明显降低,右心室功能明显改善。(3)由组织多普勒测得的右室Tei指数简便、易行、准确性高,能较全面准确地评价右心综合功能情况。Objective 1. To evaluate right ventricular function of patients with atrial septal defect and pulmonary hypertension by Tel index which were measured by tissue Doppler imaging. 2. To analyse the improvement of the right ventricular function after atrial septal defect neoplasty. Methods 28 patients with atrial septal defect and pulmonary hypertension and 33 control subjects were selected. RV Tel index was measured by tissue Doppler imaging. The pulmonary arterial systolic pressure of the atrial septal defect patients was calculated by tricuspid regurgitation velocity. Results 1. The right ventricular Tel index in patients group was significantly larger than that in the control subject;2. The right ventricular Tel index and pulmonary artery pressure in patients with atrial septal defect and pulmonary hypertension were significantly smaller than those before the neoplasty after a week. Conclusions 1. The right ventricular function of the patients with atrial septal defect and pulmonary hypertension has degraded in different degree. 2. The pulmonary artery pressure is down and the right ventricular function is improved significantly after successful neoplasty. 3. The right ventricular Tel index measured by tissue Doppler imaging which is convenient and exact can be used to estimate right ventricular global function comprehensively and exactly.
