摘  要:目的研究不同直径离体子宫肌瘤瘤体大小、消融次数、功率、时间与消融范围的关系,为临床研究奠定理论基础。方法采用微波消融治疗仪和西门子Sequoia 512超声诊断仪。选择52例手术切除的离体子宫肌瘤标本,单发和多发瘤体共计80个。在超声引导下根据瘤体大小选择不同功率、作用时间与消融次数,术后沿着微波天线插入的长轴面切开标本,分区取材作病理检查。结果不同直径瘤体与消融次数、功率、时间呈一致性关系P=0.000。<3.0cm的瘤体单次布针,3~5cm的瘤体大多立体布针,>5.0cm瘤体均为立体布针,才能彻底消融。病理变化:光镜下肌瘤细胞轮廓不清,细胞核呈碎裂状坏死,胞浆崩解呈红染,破絮状及颗粒状。或组织坏死崩解呈无结构嗜碱性改变。结论超声引导下微波消融离体子宫肌瘤的实验研究为临床研究奠定了理论基础。Objective To research the uterine myomas in vitro of various diameters with regard to the relation between their sizes, ablation frequencies, power, duration and the ablation ranges so as to lay the theoretic foundation for clinical studies. Methods Microwave ablation therapeutic apparatus and the Siemens Sequoia 512 diagnostic apparatus were applied. 52 samples of uterine myomas in vitro with 80 single and multiple masses in total were selec ted. Different frequencies, action durations and ablation times were chosen under the ultrasound guidance according to the respective tumor size. After operation,cut open the sample along long axial surface inserted with the microwave antenna for pathologic examination. Results All the tumors of various diameters had the uniform relationship with the ablating times,power and durations (P=0. 000). Thorough ablation would be possible only through the single-time electrode placement for tumors 〈3.0 cm,the stereo electrode placement for the most tumors 〈3-5 cm and the stereo electrode placement for all the tumors 〉5.0 cm. Such pathologic changes were observed: the contour of the myomatous cell was obscure under the scope,the cytoplasm was ablated and the nucleus was either shrunk, ruptured or dis- solved. Conclusions The experimental study on the uterine myomas in vitro with the ultrasound-guided microwave ablation lays a the theoretic foundation for the clinical studies.
