超声对小儿肢体原发动脉瘤样骨囊肿的诊断价值

摘　　要：目的探讨小儿肢体原发动脉瘤样骨囊肿的超声显像特征及诊断价值。方法回顾分析了28例手术证实为原发动脉瘤样骨囊肿的患儿资料,总结超声图像特征,并对照X线、CT及病理结果研究。结果小儿肢体原发动脉瘤样骨囊肿超声表现可分为2型,其中一型表现为病变部位骨皮质膨胀、变薄,髓腔内呈边界清晰的实性低回声区,无明显包膜,病灶内部回声不均匀;其另一型表现为病变部位骨皮质膨胀、变薄,髓腔内呈现不规则的多房多隔状囊实性病灶,无明显包膜,部分病例囊腔内形成"液-液"平面。彩超显示少数病灶内可见斑片状血流信号。结论超声对小儿肢体动脉瘤样骨囊肿的诊断有重要价值,可为临床诊疗提供可靠信息。Objective To study ultrasound features and its value in diagnosis on primary aneurismal bone cyst （ABC） in pediatric extremities. Methods Data of 28 cases of ABC proved by surgical operations were retrospectively analyzed. Ultrasonograms of the cases were sumarized and compared with X--ray,CT and pathologic results. Results Ultrasonagrams of pediatric ABC presented as two types. In one type,bone cortices in lesions expanded and were thin, and the intramedullary cavity presented as a sharply defined hypo-echo area without obvious envelope, and echo was not uniform in lesions. In another type,bone cortices in lesions expanded and were thin,and the intramedullary cavity pres- ented as irregular multilocular and multiseptate cystic solid lesion without obvious envelope, and liquid-liquid level ap- peared in part cases. Colour ultrasonograph showed less patches blood signals in some cases. Conclusions Uhrasonography is valuable in diagnosis of pediatric primary A.BC,and could supply reliable information for diagnosis and treatment of the lesion.
