血浆ANP对血液透析病人干体重判断的研究

摘　　要：目的：观察血浆ＡＮＰ以判断血液透析后的变化，探索用血浆ＡＮＰ来判断血液透析病人的干体重。方法：２６例慢性血液透析病人在透析前后分别测定血浆ＡＮＰ、ＣＴＲ及体重。结果：透析前后血浆ＡＮＰ为（３９５．７４±２８０．１８）ｐｇ／ｍｌ和（２２６．７８±１３０．２９）ｐｇ／ｍｌ（Ｐ＜０．００１），ＣＴＲ为（５４．６０±７．０９）％和（５２．５３±６．９７）％（Ｐ＜０．００５），体重为（５７．０３±６．０８）ｋｇ和（５５．４２±６．１５）ｋｇ（Ｐ＜０．００１），透析后患者血浆ＡＮＰ、ＣＴＲ及体重均较透析前降低。结论：提示血浆ＡＮＰ与透析病人的液体清除有关，可作为判断干体重的一个重要指标。Objective: To study whether plasma atrial natriuretic peptide (ANP) can be a marker to determine the dry weight in hemodialysis. Methods:Pre-hemodialysis and post-hemodialysis plasma ANP and cardiothoracic ratio(CTR) were evaluated in 26 chronic hemodialysis patients.Results:The levels of plasma ANP decreased with hemodialysis from(395.74±280.18)pg/ml to(226.78±130.29)pg/ml( P<0.001 ), CTR decreased from (54.60±7.09)% to (52.53±6.97)%( P <0.005),and the weight decreased from (57.03±6.08)kg to (55.42±6.15)kg( P <0.001)after hemodialysis.Conclusion Plasma ANP is useful to determine the dry weight of the chronic hemodialysis patients.
