摘 要:目的观察加味四逆泻心汤对胃癌前病变(PLGC)模型大鼠p53及细胞凋亡指数(AI)的影响,并探讨其作用机理。方法SD雄性大鼠78只,随机分为模型组、正常对照组、加味四逆泻心汤高、中、低剂量组和维酶素组。除正常组大鼠,其余5组大鼠采取综合方法复制胃癌前病变动物模型,造模第2 d起正常组、模型组给予蒸馏水3 mL灌胃;其余组分别给予加味四逆泻心汤高、中、低剂量药液及维酶素溶液灌胃;20 W后处死动物取材,光镜下观察胃粘膜病理学改变和应用免疫组织化学方法检测p53及凋亡指数(AI)基因表达情况。结果用药各组与模型组相比,癌前病变病理不同程度改善,p53基因表达下降(P<0.05),胃黏膜细胞凋亡指数明显提高(P<0.01)。结论加味四逆泻心汤高、中、低剂量组能使大鼠胃癌前病变p53基因表达减少,细胞凋亡指数增加,以中剂量为最好,表明本方通过影响p53基因和AI的含量而具有阻断胃癌前病变发生、发展的效果。Objective To observe the effects of modified Sini Xiexin decoction on experimental precancerous lesions of gastric cancer (PLGC) rats'p53 and apoptosis index (AI) and to probe into its mechanism. Methods 78 SD male rats were randomized into the model group, the normal control group, the high, medium and low doses of modified Sini Xiexin decoction groups, and vatacoenayme group. Except for the normal control group, the rest 5 groups were modeled to produce PLGC models. On the 2d after modeling, the normal group and the model group were given intragastric administration with 3mL distilled water, and the rest groups were given high, medium and low doses of modified Sini Xiexin decoction and vatacoenayme solution respectively. The rats were killed 20w later. The pathological changes of the gastric mucosa were observed under optical microscope, and the expression of p53 and AI genes were determined with immunohistochemical methods. Results Compared with the model group, the PLGC of the therapeutic groups improved in various degrees, the expression of p53 genes lowered( P 〈0.05 ), and AI increased obviously ( P 〈0.01 ). Conclusion The high, medium and low doses of modified Sini Xiexin decoction can reduce the expression of rats'PLGC p53 gene, and increase AI. The medium dose had the best effect. It is indicated that this prescription can block the occurrence and development of PLGC by affecting p53 gene and AI.
