摘 要:目的分析糖病尿患者尿白蛋白/肌酐与肾小球滤过率的相关性,了解糖尿病患者在不同肾病进程中肾小球的滤过功能。方法对77例不同年龄健康对照者及108例不同肾病期的糖尿病患者进行尿白蛋白肌酐比(UAlb/UCr)的测定,并用校正后简化MDRD方程预测肾小球滤过率。结果 1)健康对照者随着年龄的增大UAlb/UCr增加,GRF下降,但以60岁以上人群有显著性差异。相关分析显示,年龄与UAlb/UCr、GFR相关系数分别为r=0.314,P=0.005、r=-0.506,P=0.000;UAlb/UCr与GFR无相关性。糖尿患者年龄与UAlb/UCr、GFR相关系数分别为r=0.363,P=0.000、r=-0.477,P=0.000;UAlb/UCr与GFR呈负相关r=-0.374,P=0.000。经偏相关分析调整年龄后,UAlb/UCr与GFR仍为显著负相关r=-0.232,P=0.014。2)糖尿病不同肾病组UAlb/UCr、GFR的比较,UAlb/UCr随着糖尿病肾病的加重逐渐增高。GFR:B组明显高于其他各组,C组开始下降。结论糖尿病肾病早期肾小球滤过功能处于高滤过状态,到临床蛋白尿期肾小球滤过功能开始下降。预示着终末肾病的发生。Objective To analyze the association of urinary albumin/ereatinine and glomerular filtration rate in patients with Diabetes mellitus,and to study the filtration function of glomeruli in diffenrent stages of diabetic nephropathy. Methods Urinary albumin/creatinine(UAlb/UCr) was detected in 77 subjects of diffenrent ages in healthy control group and 108 patients with diffen- rent stages of diabetic nephropathy,glomerular filtration rate was predicted via simplified MDRD equation after adjustment. Results 1)UAlb/UCr raised while GRF decreased as ages in control group, significant difference was observed in subjects above 60 years old(r=0. 314 ,P= 0. 005 for UAlb/UCr; r= 0. 506, P = 0. 000 for GRF respectively) ~ Correlation was not found between UAlb/ UCr and GFR. correlation coefficient of age and UAlb/UCr,GFR in patients with diabetes was r=0. 363 ,P=0. 000,r=0. 477 ,P= 0. 000 respectively;negative correlation was found between UAIb/UCr and GFR(r=- 0. 374, P= 0. 000). After ages were adjusted by partial correlation analyses, significant negative correlation was still observed(r = - 0. 232, P = 0. 014). 2) Comparison of UAlb/ UCr,GFR among diffenrent stages of diabetic nephropathy, UAIb/UCr was increased with depravation of diabetic nephropathy. GFR in Group B was much higher than other groups and began to decrease in Group C. Conclusion Filtration function of glomeruli in early stages of diabetic nephropathy was in high filtration condition;Filtration function of glomeruli was not decreased until clini cal alburninuric stage occurred. Development of terminal nephropathy was predicted.
