椎弓根钉棒系统内固定治疗胸腰椎爆裂性骨折36例围术期护理

摘　　要：目的：探讨椎弓根钉棒系统内固定治疗胸腰椎爆破性骨折围术期的护理方法。方法：采用后路减压椎弓根钉棒系统内固定治疗胸腰椎爆裂性骨折患者36例，围手术期给予精心护理，并定期随访并评定神经功能恢复情况。结果：所有患者手术经过顺利，术后伤椎椎体前缘高度及Cobbs角较手术前明显恢复（P〈0．01）。随访8～26个月，患者均能下地行走，X线摄片显示36例均骨性融合。围手术期病情稳定，无并发症发生。结论：后路减压、椎弓根钉棒系统内固定治疗胸腰椎爆裂性骨折，能有效恢复椎体高度、生理弯曲及神经功能。围术期相关护理及健康教育能够保证该项技术的顺利实施。Objective ： To investigate the nursing methods in the perioperative period in the treatment of burst thoracolumbar fractures by internal fixation with pedicle screw system. Methods： 36 patients with burst thoracolumbar fractures received internal fixation with pedicle screw system via posterior decompression and were given careful perioperative nursing care. The regular follow - up and assessment of recovery of neurological function were conducted. Results： The procedures of all operations were smooth. The heights of anterior border and kyphosis Cobbs＇angle of compressed vertebral bodies obviously restored after surgery （ P 〈0.01 }. The patients could walk by follow - up for 8 to 26 months and X - ray showed 36 patients got bone fusion. The patients＇conditions were stable in perioperative period and no complications occurred. Conclusion： The internal fixation with pedicle screw system via posterior decompression can effectively restore vertebral height, curvature and neurological function in the treatment of burst thoracolumbar fractures. The appropriate perioperative nursing care and health education can ensure the smooth implementation of the technology.
