摘  要:目的研发可用于隆鼻手术术前设计、手术模拟及假体制作的计算机三维模拟软件系统。方法提取20例单纯隆鼻术求美者手术前后的头部螺旋CT数据。应用DICOM格式数据进行三维重建和图像分割,分析手术前后面部轮廓的变形情况,建立数学模型。应用三维数字建模方法和软组织虚拟变形技术,开发计算机三维虚拟隆鼻手术系统。使用此系统预设隆鼻效果、模拟隆鼻手术,并将获取的植入体参数用于假体模型的制作,应用于实际的临床手术。结果建立了鼻假体与鼻外形改变之间关系的数学模型,经过20例隆鼻实例数据的校验,证明具有很高的精度。结论本课题组研发的Pureplastic虚拟整形系统,能够完成隆鼻手术的术前设计及术后效果模拟,并提供植入体参数用于假体制作,证实了计算机技术辅助隆鼻术的可行性。Objective To develop a computerized three-dimensional virtual augmentation rhinoplasty system for preoperative design, surgical simulation and prosthesis production. Methods Twenty subjects underwent augmentation rhinoplasty, the cranial computed tomography scanning was performed before and one month after surgery. Three-dimensional model was then reconstructed based on their DICOM data. The mathematical model of the facial soft tissue deformation was established and improved based on the analysis of the facial contour deformation before and after operation. The computerized three-dimensional simulation system was established for surgical implant design and model making. Results The mathematical model of relationship between nose and nasal prosthesis shape was established and the model was proved to be highly precise by the validation of 20 cases. Conclusion The virtual plastic system Pureplastic developed in the study can simulate the process of augmentation rhinoplasty, which can be used in the surgical design and prosthesis making.
