摘 要:目的:探讨电针次髎对逼尿肌反射亢进的效应及作用机理。方法:成年雌性SD大鼠44只,脊髓横断术后纳入37只,分为手术后电针组16只、手术后模型组16只、正常组(假手术组)5只,电针组于大鼠出现尿失禁后给予电针双侧次髎,20Hz连续波每日1次,每次2h,共治疗14次,模型组、正常组不治疗,3组均于首次治疗前、末次治疗结束后进行尿流动力学检测,于末次疗后用免疫组化染色法观察辣椒素受体VR1表达的情况。结果:逼尿肌反射亢进大鼠(电针组和模型组)的最大膀胱内压升高(P<0.05),最大膀胱顺应性降低(P<0.05),骶髓排尿中枢VR1表达增多(P<0.05)。治疗后,电针组的最大膀胱内压明显降低(P<0.05),与正常组无显著差异(P>0.05);模型组的最大膀胱内压亦明显降低(P<0.05),但与正常组有显著差异(P<0.05)。疗后电针组的最大膀胱顺应性明显升高(P<0.05),模型组的最大膀胱顺应性无明显变化(P>0.05)。疗后电针组骶髓排尿中枢VR1表达比模型组少,比正常组多,差异均有统计学意义(P<0.05)。结论:电针"次髎"穴能抑制脊髓横断大鼠的膀胱活动过度状态,可以降低骶髓排尿中枢VR1的表达,表明C纤维活动减弱,这可能是电针"次髎"治疗逼尿肌反射亢进的机制之一。Objective:To observe the effect and mechanism of electroacupuncture at"BL 32"on detrusor hyperreflexia.Methods:Forty-four adult female Sprague-Dawley rats were selected,but only thirty-seven were included into the trial after spinal cord injury(SCI),from which sixteen were randomized into treatment group,sixteen into control group and five into normal group(sham surgery group).Electro-acupuncture(20Hz,continuous wave)at bilateral"BL 32"were given to treatment group after incontinence occurred for 2 hours for 14 times.The other two groups were not given any treatment.All groups were accepted urodynamics detection before the first and after the last treatment,and accepted immunohistochemical staining to observe the VR1 impression after the last treatment.Results:The maximum bladder pressure(MBP)increased in rats with DH(electro-acupuncture group and model group)(P0.05),the maximum bladder compliance(MBC)decreased(P0.05)and the VR1 expression increased in the sacral micturition center after SCI(P0.05).After treatment,the MBP of electro-acupuncture group significantly decreased(P0.05),having no difference with normal group(P0.05);the MBP of model group significantly decreased(P0.05),but having significant difference with normal group(P0.05).The MBC in electro-acupuncture group increased significantly(P0.05),but no change in model group(P0.05).The VR1 expression in sacral micturition center was less in the electro-acupuncture group than in the model group but more than the normal group(P0.05).Conclusion:Electro-acupuncture at "BL 32" can inhabit the over-activity of bladder in SCI rats and reduce the VR1 expression in the sacral micturition,which showed that the C-fiber's activity was weakened,which maybe one of the mechanisms of EA at "BL 32" in treating DH.
