包块型输卵管间质部妊娠腹腔镜手术方法探讨

摘　　要：目的:探讨腹腔镜下缝扎法切除包块型输卵管间质部妊娠的手术方法。方法:22例包块型输卵管间质部妊娠均采用腹腔镜手术。3例有生育要求者行剖开取胚术;包块2～4cm者10例,无生育要求者包块基底部行双"U"字缝合后切除患侧输卵管及部分宫角;包块>4cm者共9例,均行患侧输卵管及部分宫角切除。比较3种手术方法的手术情况、再次妊娠结局、预后。结果:22例均在腹腔镜下完成患侧输卵管切除加部分宫角楔形切除,无一例中转开腹和子宫切除,术中出血、手术时间、术后血β-HCG恢复正常时间比较,差异无统计学意义(P>0.05)。22例患者中18例随访2～36个月,子宫均正常大小。结论:采用缝扎法切除患侧输卵管及部分宫角是治疗包块型输卵管间质部妊娠的有效方法。Objective：To investigate the laparoscopic operation method for resection of lump type interstitial tubal pregnancy by suture.Methods：22 lump type interstitial tubal pregnancy patients were treated by laparoscopic operation.3 patients with fertility requirement were treated by resection of oviduct and removal of embryo.10 patients without fertility requirement and with lump diameter from 2~4 cm were treated by double ＂U＂ suturing basilar part of lump then cutting affected side of oviduct and part of uterine corner.9 patients with lump diameter4cm were treated by cutting affected side of oviduct and part of uterine corner.The operation information,subsequent pregnancy outcome and prognosis of these three methods were compared.Results：Resection of pregnancy interstitial tube and wedge resection of part of uterine corner by lapa-roscope were finished in 22 patients.No one underwent conversion to laparotomy and hysterectomy.There was no significant difference about operation bleeding volume,operation time and the time of β-HCG recovery after operation in different types（P0.05）.18 of 22 patients followed up for 2 to 36 months,and the size of the uterus was normal.Conclusions：It is a utility method to treat lump type interstitial tubal pregnancy by suture and resection of pregnancy interstitial tube and part of uterine corner.
