乳腺癌术后抑郁障碍中医量表的研制

摘　　要：目的:编制符合我国国情、具有中医内涵的乳腺癌术后抑郁障碍中医量表。方法:通过文献调研、专家咨询、预试验和研究小组讨论形成条目池,对150例乳腺癌术后患者(抑郁障碍患者100例和非抑郁患者50例)临床试用后,进行条目筛选和量表考评。结果:文献调研结果的76个条目,经预试验、咨询专家和小组讨论,形成心理、功能、躯体3个维度、57个条目的预调查表。临床试用后,经条目筛选修订的量表包括3个维度、36个条目。量表考评结果为可行性100%;重测信度0.949、Cronbach’a系数为0.717;以HAMD金标准的效标效度0.502,因子分析结构效度与量表的理论构想基本相符合;在乳腺癌术后抑郁和非抑郁患者中以及中医团体情志疗法干预前后量表总分和各维度均有显著性差异(P<0.01)。结论:乳腺癌术后抑郁障碍中医量表具有良好的效度、信度和反应度,为中医药的临床疗效评价提供了一个有效的工具。Objective：To develop the TCM scale of depression after breast cancer,which is corresponding to China ,s national conditions and with TCM connotation.Methods：Establishing item pool through the literature research,expert advice,pre-testing and study groups,discussion.Clinical trial using on 50 breast cancer patients with surgery（100 depressive disorders and 50 non-depressive disorders）were used to build the item Selection and scale evaluation.Results：The thick table constitutes of 76 items by literature investigation.The pre-survey TCM scale of depression after breast cancer includes three fields and 57 items formed by pre-testing,consulting experts.After the item selection in the clinical trial,the amendmentsed scale includes three dimensions,36 items.The evaluation results of the scale：feasibility is 100%;test-retest reliability is 0.949;Cronbach＇a coefficient is 0.717;with HAMD gold standard criterion validity is 0.502,factor analysis of construct validity and scale structure consistent with the basic theoretical concepts.The scale scoring of psychologicao dimension,functional dimensions,body dimensions and total score differences is significant in depression in breast cancer and nondepression（P0.01）.After the TCM group emotional therapy intervention,the total and the dimensions before treatment is significantly lower（P0.01）.Conclusions：The TCM scale of depression after breast cancer has good reliability,validity and the responsiveness,which is one of the effective tools for evaluating TCM clinical treatment effectiveness in the future.
