摘 要:目的:探讨应用双能量数字减影摄影术(DES)提高对下肢静脉疾病的诊断准确性。方法:50例患者应用双能量减影技术行下肢静脉造影,25例初诊为下肢静脉疾病的患者为疾病组,25例健侧下肢为对照组。比较注射对比剂2min及4 min时2组内及2组间减影前后图像上软组织与下肢静脉血管内灰度值的差值;比较两种图像在注射对比剂后4min时股深静脉瓣的检出率。结果:疾病组中,注射对比剂2 min及4 min时减影前软组织与静脉血管平均灰度差分别为-110±25及-506±36;减影后分别为312±12及793±22。对照组中,减影前注射对比剂2 min及4 min时软组织与静脉血管的平均灰度差分别为-549±31及-665±38,减影后分别为684±24及836±28。2组内及2组间同时间段内软组织与静脉血管内的灰度差均有明显差异(P<0.01)。减影前软组织与血管内对比剂的灰度差较小,减影后明显增加。疾病组中,注射对比剂4 min后,减影前常规DR摄片对股深静脉瓣的检出率为68%,减影后DES骨像上股深静脉瓣的检出率为92%。结论:下肢静脉疾病减影前后图像的对比度有显著改变,DES能提高下肢静脉疾病诊断准确性。两次曝光双能量减影法应用在下肢静脉疾病诊断中能有效提高诊断效果。Objective:To improve the accuracy of the diagnosis in lower limb venography with dual energy digital subtraction photography (DES). Methods:Of 50 cases with lower limb venography using DES,25 cases were initially diagnosed as lower limb vein disorders and counted for disease group,the other 25 cases were control group with the normal lower limb. After injection of contrast medium, the differences of the gray scale between soft tissue and enhanced lower limb veins on the images of disease group and control group before and after subtraction were compared. The detection rate of deep femoral vein valve at 4 rainutes after contrast injection in these 2 groups were compared as well. Results: In the disease group,at 2min and 4rain after contrast injection,the average gray level differences of the soft tissue and enhanced vein were (- 110±25) and ( -506±36) respectively before subtraction,and were (312±12) and (793±22) after subtraction. In the control group,the average gray level differences of the soft tissue and enhanced vein were ( -549±31) and (-665±38) respectively before subtraction, and were (684 ± 24) and (836 ± 28) after subtraction. There were significant statistical differences of the gray scale between soft tissue and enhanced lower limb veins both within the same group and at the same time period before and after subtraction (P〈0. 01). The gray scale difference between soft tissue and contrast enhanced blood vessels was lower before subtraction, but much higher after subtraction. In the disease group, the detection rate of deep femoral vein valves at 4min after contrast injection was 68% on the conventional DR before subtraction and was 98 % on DES bone image after subtraction. Conclusion: The contrast of images of lower limb vein disorders showed significant improvement before and after subtraction, the accuracy of the diagnosis of lower limb vein disorders could be heightened by DES, the diagnostic efficacy of lower limb vein disorders could be ra
