鼻咽癌放疗后患者中医证型研究

摘　　要：目的探讨鼻咽癌放疗后患者的中医证候学规律和特点。方法采用临床流行病学原则和方法,纳入鼻咽癌放疗后患者94例,收集临床中医证候信息,构建证候信息数据库,对四诊指标进行聚类分析,再进行鼻咽癌放疗后中医证型相关因素分析。结果通过聚类分析,得出6种鼻咽癌放疗后中医证型。不同年龄阶段患者各证型所占比例比较差异无统计学意义(P>0.05);不同放疗后时间段在肺胃阴虚证、肾阴亏虚证、脾胃虚弱证分布有统计学意义(P<0.05);不同放疗方式在肺胃阴虚证、脾胃虚弱证分布有统计学意义(P<0.05)。结论鼻咽癌放疗后中医证型为热毒伤阴、脾胃虚弱、痰热困结、肺胃阴虚、肾阴亏虚、痰瘀互结6种,中医证型分布与不同放疗后时间、不同放疗方式有关。Objective To explore the law and characteristic of TCM syndrome type of nasopharyngeal carcinoma after radiotherapy.Methods The methods of clinical epidemiology were used in the enrollment of 94 cases. Their information of clinical TCM syndromes was collected. The syndrome information database was established for the cluster analysis on the diagnostic indices and the correlated factor analysis on the TCM syndrome types of nasopharyngeal carcinoma after radiotherapy.Results With the cluster analysis,there were 6 TCM syndrome types of nasopharyngeal carcinoma after radiotherapy achieved. There was not significant difference in the occupying percentage of TCM syndromes in different age stages （P〉0.05）. In the different time periods after radiotherapy,there was significant difference in the distribution of syndromes of yin-deficiency of the lung and stomach,yin-deficiency of the kidney,and weakness of the spleen and stomach （P〈0.05）. With the different radiotherapy methods adopted,there was significant difference in the distribution of syndromes of yin-deficiency of the lung and stomach and weakness of the spleen and stomach （P〈0.05）.Conclusion After radiotherapy,there exist 6 TCM syndrome types of nasopharyngeal carcinoma,including the injury of yin by evil heat toxins,weakness of the spleen and stomach,phlegm and heat in combination,yin-deficiency of the lung and stomach,yin-deficiency of the kidney,and phlegm and blood stasis in combination,the distribution of which is related with the different time periods after radiotherapy and the different radiotherapy methods.
