老年食管癌病人围手术期的心理状态特点及其对策

摘　　要：目的探讨分析老年食管癌患者围手术期的心理状态及制定对策。方法对60例老年食管癌手术病人于术前1 d和术后1周分别用抑郁自评量表(SDS)和焦虑自评量表(SAS)进行检测,并与全国常模比较,针对影响因素施行医疗、护理相结合的措施。结果 (1)手术前老年食管癌患者的SDS和SAS分值表明,患者术前有较明显的焦虑、抑郁情绪;(2)手术后,经采取医疗、护理相结合的治疗手段,老年食管癌患者的SDS和SAS分值与手术前相比,显著降低(P<0.01),恢复至全国常模水平(P>0.05)。采取医疗、护理相结合的治疗手段,可取得较满意的效果。结论围手术期老年食管癌手术病人存在较明显的焦虑、抑郁情绪。根据原因分析,实施有效的医疗、护理相结合的治疗可有效地改善患者的心理状态,有利于患者早日康复。Objective To discuss the mental state of perioperative old patients of esophageal carcinoma and countermeasures for them.Methods 60 patients received esophageal carcinoma operation were investigated with Self-Rating Depression Scale（SDS） and Self-Rating Anxiety Scale（SAS）.Compared with the norms of our country at last.Aim directly at the influencing factor to carry out cure-nurse binding countermeasures for them.Results（1）The scores of SDS and SAS in the two group patients in pre-operation were indicated that patients had evident depression and/or anxiety symptoms pre-operation.（2）Compared with pre-operation,the scores of SDS and SAS in the intervention group patients showed significantly lower after operation（P0.01）,which returned to the norms of our country（P0.05）.This indicated that carry out cure-nurse binding countermeasures for them could obtain satisfaction effect.Conclusion The perioperative patients of esophageal carcinoma had obvious depression and/or anxiety symptoms,Cure-nurse binding countermeasures could improve their mental status efficiently,this was helpful for patients to recover early.
