对我院鲍曼不动杆菌感染使用抗菌药物的疗效分析

摘　　要：目的:通过调查鲍曼不动杆菌培养阳性病例,分析抗菌药物使用及治疗效果情况。方法:检索我院2008年1月～2009年1月细菌培养鲍曼不动杆菌阳性病例70例,调查鲍曼不动杆菌对抗菌药物的敏感性、临床抗菌药物使用及治疗效果情况。结果:在常用抗菌药物中,亚胺培南的敏感率最高为84.3%,替卡西林/克拉维酸次之为37.1%,其余药物敏感率不足23%。而临床针对鲍曼不动杆菌使用前5位的抗菌药物分别为亚胺培南、头孢吡肟、哌拉西林/他唑巴坦、头孢哌酮/他唑巴坦和头孢米诺。根据药敏试验结果选用抗菌药物的依从率为39.7%,治疗的有效率为25.8%。结论:鲍曼不动杆菌耐药情况严重,临床依据药敏试验结果选用抗菌药物的依从性较低,治疗疗效欠佳,因此需加强抗菌药物应用管理工作。OBJECTIVE： To analyze the application and therapeutic effect of antibacterials by investigating the positive cases of Acinetobacter baumannii. METHODS： 70 positive cases of Acinetobacter baumannii were retrieved from our hospital during Jan. 2008 Jan. 2009. The susceptibility of Acinetobacter baumannii to antibacterials, the application and therapeutic effect of antibacterials were investigated. RESULTS： Among common antibacterials, the susceptibility of imipenem was the highest （84.3%）, followed by ticarcillin/clavulanic acid （37.1%）. And the others were less than 23%. Top 5 frequently used antibacterials to treat Aci- netobacter baumannii were imienem, cefepime, piperacillin/tazobactam, cefoperazone/tazobactam and cefminox. The compliance rate of selected antibacterials according to drug sensitivity test was 39.7%. Total effective rate was 25.8%. CONCLUSION： Antibacterial resistance of Acinetobacter baumannii is severe. Selected antibacterials according to drug sensitivity test aren＇t easy to take and be accepted by patients. Therapeutic effect isn＇ t satisfactory, so we should enhance management of antibacterials use.
