陈旧性心肌梗死患者心室晚电位检测及其与室性心律失常发生的关系

摘　　要：目的 :探讨陈旧性心肌梗死患者心室晚电位 (VLP)的检出率及其与室性心律失常发生的关系。方法 :对 185例陈旧性心肌梗死患者、 2 2 5例无心肌梗死的冠心病患者以及 72例正常对照组分别进行心室晚电位检测。结果 :心肌梗死组VLP阳性率明显高于冠心病组及正常对照组 (P <0 0 1);无心肌梗死的冠心病患者VLP阳性率高于正常对照组 ,但差异无统计学意义 ( P >0 0 5 );在心肌梗死组 ,下壁心肌梗死患者VLP阳性率高于前壁梗死患者 ,差异亦无统计学意义 (P >0 0 5 );心肌梗死组伴有室性心律失常患者VLP阳性率明显高于不伴室性心律失常患者 ( P <0 0 1)。结论 :不同部位陈旧性心肌梗死患者的心室晚电位阳性率无显著性差异 ,但是心室晚电位与心肌梗死后室性心律失常的发生密切相关 ,心室晚电位检测结果对于评价陈旧性心肌梗死患者预后有重要的临床意义。Objective:To investigate the positive rate of ventricular late potentials(VLP) and its relationship with ventricular arrhythmias in patients with old myocardial infarction.Methods:Ventricular late potentials were measured in one hundred and eighty five cases with old myocardial infarction and two hundred and twenty five coronary heart disease patients without myocardial infarction and seventytwo normal subjects.Results:The positive rate of VLP in patients with myocardial infarction was obviously higher than in coronary heart disease patients and normal control subjects(21 08% versus 10 67%,21 08% versus 5 56%,P<0 01).The positive rate of VLP in coronary heart disease patients was higher than in normal control subjects,but the difference had no statistics significance(10 67% versus 5 56%,P>0 05).The positive rate of VLP in patients with inferior wall myocardial infarction was higher than in patients with anterior wall infarction,the difference had also no statistics significance(25 37% versus 15%,P>0 05).Among the patients with myocardial infarction,the positive rate of VLP in patients with ventricular arrhythmias was remarkably higher than in those without ventricular arrhythmias(53 85% versus 18 60%,P<0 01).Conclusions:The results suggest that the positive rate of VLP in patients with different regions of infarction had no significant difference.The ventricular late potential correlated closely with the incidence of ventricular arrhythmias after myocardial infarction.The meaurement of VLP has important clinical significance for evaluating the prognosis of patients with old myocardial infarction.
