围术期C-反应蛋白与剖宫产术后切口愈合的关系探讨

摘　　要：目的研究剖宫产围手术期产妇血清中C-反应蛋白(CRP,C-reactive protein)的动态变化,探讨围术期C-反应蛋白与剖宫产术后切口愈合的关系。方法以2009年2～6月该院实施剖宫产术的患者共160例,按切口愈合情况分为切口甲级愈合组144例,切口愈合不良组16例(其中乙级愈合11例,丙级愈合组5例)。分别于术前,术后6、24、48、72及120h测定血清CRP,同时测定病人术前,术后24、48及120h的血常规。结果术前切口愈合不良组CRP均值及术后24和48h切口愈合不良组CRP均值以及血WBC均值均较切口甲级愈合组明显增高,差异有显著性(P<0.05),两组术后6和72hCRP均值、术后120h CRP均值以及术前及术后120h血WBC均值均差异无显著性(P>0.05)。结论围术期CRP动态变化可以作为剖宫产术后腹壁切口愈合的监测指标,在术前和术后早期均有一定的预测价值。【Objective】The purpose of this study was to determine the relationship of CRP （C-reactive protein） serum concentrations with incision wound healing after cesarean sections.【Methods】One hundred and sixty women undergoing cesarean delivery during the period from February to June in 2009 were allocated into 2 groups：One hundred and forty-four patients with the first grade wound healing belonged to group 1; while in group 2 there were 16 cases of incision wound malunited （11 patients with the second grade wound healing,5 patients with the third wound healing）. Determined separately the serum concentrations of CRP in parturient before the cesarean section and 6,24,48,72 and 120 h after the cesarean section. Tested the routine blood examination of the patients before the cesarean section and 24,48 and 120 h after the cesarean section. 【Results】The CRP serum concentrations be-fore the cesarean section and 24 h,48 h after the cesarean section,and the blood value of WBC 24 h,48 h after the cesarean section of group 2 were demonstrated significantly higher than those of group 1 （P〈0.05）,while no signifi-cant differences between the two groups were showed at the CRP serum concentrations 6h,72h,120 h after the ce-sarean section and the value of WBC before the cesarean section and 120h after the Cesearean section （P〈0.05）. 【Conclusions】The dynamic serum concentrations of CRP in perioperative parturient may be regard as a marker to forecast wound healing before and early after the cesarean section.
