经膀胱造瘘口超声与气压弹道膀胱碎石术29例报告

摘　　要：目的探讨经膀胱造瘘口超声与气压弹道碎石术治疗膀胱结石的效果。方法2006年1月～2008年10月吉林大学第二医院与吉林大学中日联谊医院收治膀胱造瘘术后膀胱结石患者29例,通过膀胱造瘘口肾镜或输尿管镜下应用超声与气压弹道碎石,8例合并前列腺增生(BPH)患者同期行经尿道前列腺电切术(TURP)。结果所有手术均顺利完成,手术时间30～180min,平均55min。无大出血、膀胱穿孔、结石残留等。TURP8例术中无电切综合征。所有患者均获得随访,随访时间1～24个月,平均6个月。2例患者术后15个月再发膀胱结石,再次行经膀胱造瘘口镜下超声与弹道碎石术。结论对于膀胱造瘘术后膀胱结石患者,通过造瘘口肾镜或输尿管镜下超声与气压弹道碎石,属于微创手术,效果满意。但其作为一种新开展的手术方法,要求成熟的有经验的医师操作,术中注意保护膀胱黏膜,术者应能熟练掌握腔镜操作,灵活地处置各种情况。【Objective】To investigate treatment effect of bladder stones with ultrasound and pneumatic lithotripsy after cystostomy. 【Methods】Twenty-nine cases with bladder stones were treated with ultrasound and pneumatic lithotripsy after cystostomy from January 2006 to October 2008 in China-Japan Union Hospital of Jilin University. Eight cases with benign prostatic hyperplasia （BPH） underwent transurethral resection of the prostate （TURP） at the same time.【Results】All the operations succeeded. The operation time was 30 to 180 minutes （mean 55 min）. There was no case of hemorrhage, perforation or remaining stones. None of the 8 cases with TURP had transurethral resection syndrome。The follow-up was 1 to 24 months （mean 6 months）. Two cases had recurrent bladder stones and underwent the operation again. 【Conclusion】The therapeutic effect of ultrasound and pneumatic lithotripsy after cystostomy is satisfied. It belongs to minimally invasive operations and must be done by skillful doctors.
