艾滋病患者血清免疫球蛋白及补体的变化及其临床意义

摘　　要：目的测定AIDS患者抗病毒治疗前、后体内血清免疫球蛋白及补体水平,并探讨其临床意义。方法采用免疫散射比浊法测定50例未治疗患者及55例HARRT治疗患者血清免疫球蛋白IgG、IgM、IgA和补体C3水平,同时选取30例健康体检者为对照。结果未治疗组患者除IgM外,IgG、IgA水平均显著高于健康对照,补体C3则低于健康对照,差异具统计学意义(P<0.05);治疗组患者IgG、IgM、IgA水平较未治疗组显著下降,而补体C3显著升高,差异均具统计学意义(P<0.05);IgG、IgM、IgA水平与CD4+T细胞计数显著负相关,与病毒载量正相关,而补体C3则与CD4+T细胞计数正相关,与病毒载量负相关。结论 AIDS患者体液免疫功能紊乱,血清免疫球蛋白及补体变化与病情明显相关,动态测定免疫球蛋白及补体水平,对于判断病情、衡量治疗效果和预后的估计有重要价值。【Objective】To study the serum levels of immunoglobulin and complement C3 in AIDS patients before and after highly active antiretroviral therapy（HARRT）,and furthermore to explore its clinical significance.【Methods】Serum concentration of immunoglobulin and complement C3 of 50 antiretroviral-na ve patients and 55 patients on HARRT were measured by immunoturbidimetry,at the same time,30 healthy persons were chosen as control group.【Results】The results showed that excepting IgM,the levels of IgG and IgA were significantly higher and C3 was lower in antiretroviral-na ve AIDS patients than in healthy controls（P 0.05）.The levels of IgG,IgM and IgA in the HARRT group decreased and C3 increased significantly（P 0.05） when compared with antiretroviral-na ve group.The levels of IgG,IgM and IgA were in negative correlation with the number of CD4^＋T cells and positive correlation with viral load,but C3 was in positive correlation with CD4^＋T cell number and negative correlation with viral load.【Conclusion】The humoral immunity in AIDS patients is obviously in serious disorder.And changes of serum immunoglobulin and complement are evidently correlated with disease stage,so dynamic surveillance on immunoglobulin and complement could be beneficial to evaluation of the disease condition,treatment effect and prognosis in AIDS.
