碘造影剂不良反应的预防、护理及急救处理

摘　　要：目的探讨碘造影剂不良反应的预防、护理及急救处理的方法。方法我科在2004年1月至2009年10月对4500例静脉肾盂造影及CT增强检查患者碘造影剂所致的不良反应进行回顾性分析,其中使用离子型造影剂280例,非离子型造影剂4220例。结果共出现86例不良反应,其中轻度不良反应65例,中度不良反应18例,重度不良反应3例。经对症治疗,静脉注射地塞米松,肾上腺素皮下注射,病情得到有效缓解、好转。结论充分做好造影剂使用前的准备,提倡使用非离子型造影剂,加强造影前、中、后的护理是预防和处理不良反应发生的有效措施。Objective To study the methods of pervention,nursing care and emergency manage-ment in the adverse reaction of iodine contrast medium.Methods From January 2004 to October 2009,4500 cases received examination of IVP and CT emhancement inducing 86 cases with adverse reaction were analyzed retrospectively.Results In this group,the adverse reaction occurred in 86 cases.Among them,mild,moderate and severe adverse reaction were in 65,18 and 3 cases respectively.Via Symptomatc treatment,including intravenous dexamethasone and adrenaline Subcutaneous imjection,the patients condition was relieved and improved effectively.Conclusion An enough preparation for using contrast medium,advocating use of nonionic contrast medium,enhancing nursing care before,during and after contrast examination are effective measures for preventing and managing adverse reaction.
