高血压左室肥厚病人的肾功能评价

摘　　要：目的研究高血压伴左室肥厚患者肾功能的改变。方法对３０例高血压伴左室肥厚病人及２１例年龄相当的健康人对照组行核素动态肾功能检测及彩色多普勒超声肾血流动力学检查，测定肾小球滤过率（ＧＦＲ）和肾脏指数（ＲＩ）及肾内动脉分布，段动脉、叶间动脉的阻力指数（ｒｉ），血流峰速加速时间（ＡＴ），收缩期最大血流速度（ＶＳ），舒张期末最低血流速度（ＶＤ）。结果观察组肾内弓形动脉、小叶间动脉较对照组明显减少甚至不能显示，肾内段动脉和叶间动脉的阻力指数（ｒｉ）显著增加（Ｐ＜０．００１），ＡＴ明显延长（Ｐ＜０．００１），ＧＦＲ显著降低（Ｐ＜０．００１），ＶＳ，ＶＤ及肾脏指数（ＲＩ）亦明显降低（Ｐ＜０．０１）。结论当临床血肌酐、尿素氮均正常时，高血压病伴左室肥厚的很多患者已经有肾功能的改变。Aim\ To observe the change of the renal function in patients with esential hypertension and left ventricular hypertrophy.\ Methods\ Radionuclide ambulatory renography and color Doppler renal hemodynamics were studied in 30 patients with esential hypertension and left ventricular hypertrophy and 21 healthy control group. The glomerular filtration rate(GFR),renal index(RI) and the distribution of intrarenal artery were investigated.The resistive index(ri), the systolic peak velocity(VS),andthe low diastolic flow velocity(VD) and acceleration time(AT) of the intrarenal arterial flow(AT) were measured.\ Results\ Compared with control group the density of intrarenal artery of patients with essential hypertension and left ventricular hypertrophy were significantly decreased. In some patients, it even could not be displayed..The resistive index(ri) values of the segmental artery and the interlobar artery were significantly higher than those of the control group( P <0.001). AT were significantly increase( P <0.001);GFR were significantly lower( P <0.001); VS,VD and renal Index(RI) were significantly lower( P <0.01).\ Conclusion\ Change of the renal function was found in patients with essential hypertension and left ventricular hypertrophy even when the serum creatinine and blood urea nitrogen were normal.For preventing the progress of hypertension and guiding the treatment,it is very useful to measure and found the change of renal function in early phase.
