Bentall术治疗主动脉根部瘤21例分析

摘　　要：目的总结Bentall手术治疗主动脉根部瘤的临床经验。方法采用Bentall手术治疗主动脉根部瘤21例,其中主动脉瓣关闭不全伴升主动脉瘤样扩张13例,马凡综合征5例,大动脉炎1例,DeBakeyⅡ型主动脉夹层2例。术前心功能分级（NYHA）Ⅱ级12例,Ⅲ级9例。同期行二尖瓣置换术3例,三尖瓣成形术2例,冠状动脉搭桥术2例。结果全组无手术死亡。术后并发症包括低心排综合征2例,严重心律失常1例,二次开胸止血1例。术后随访21例均恢复良好,心功能Ⅰ~Ⅱ级。结论 Bentall手术是治疗主动脉根部瘤的首选方法。良好的缝合技术和较短的体外循环时间有利于提高手术成功率。Objective To summarize the experience of Bentall operation for aortic root aneurysm.Methods 21 patients with ascending aortic root aneurysm underwent Bentall operation,including 13 cases of ascending aortic aneurysm and aortic valve regurgitation,5 cases of Marfan syndrome,1 case of Takayasu arthritis,and 2 cases of acute dissecting aortic aneurysm.Preoperatively,12 cases were in NYHA function class Ⅱ and 9 cases in class Ⅲ.Concomitant operations included mitral valve replacement（3 cases）,tricuspid valve annuloplasty（2 cases） and coronary artery bypass grafting（2 cases）.Results There was no operative death.The postoperative complications included low cardiac output（2 cases） and severe arrhythmia（1 case） and reoperation for hemostasis following open-heart surgery（1 case）.Follow-ups showed that all the cases were successfully recovered,with heart function in class Ⅰ-Ⅱ.Conclusion Bentall procedure should be the optimal option for ascending aortic root aneurysm,and furthermore,excellent surgical suture techniques in combination with reduced cardiopulmonary bypass（CPB） time are the essential factors to successful surgery.
